EE was assisting with removal of pin from Track hoe bucket and attempted to catch it as it came through the bushing. Pin was too heavy to be handled and fell, crushing his finger against bucket, bone fracture to last joint of 3rd digit of left hand.

Showing all 11Employee was fabricating a rock chute on ground. He had rolled chute over to access other side. Chute then rolled back on his right foot breaking his first 3 toes just behind the steel toe guard in his boot.
Employee was removing rock from crusher and pulled muscle in his lower back.
TWO BEARING CAPS HAD BEEN REMOVED FROM THE CRUSHER/JAW. THEY WERE PLACED ON A PALLET. THE MINER WAS ASSIGNED TO CLEAN THE CAPS IN PREPARATION TO PUT IN NEW BEARINGS AND GREASE. AS HE WAS TURNING ONE OF THE CAPS OVER TO CLEAN IT, IT SLIPPED WHILE HE WAS HOLDING IT OVER STRIKING HIS RIGHT HAND BETWEEN THE TWO CAPS.
EE was removing a large stone from the haul road from the pit area which was too large to lift, by moving rolling it out of the roadway and sprang wrist on left hand.
EE WAS REPAIRING TAIL PULLEY ON CONVEYOR. THE TAIL PULLEY WAS REMOVED AND EE ATTEMPTED TO LIFT IT WHEN IT ROLLED AND TORE EE'S FINGERNAIL.
EE LIFTING A WELDING GAS CYLINDER WITH ANOTHER EE WHEN PAIN FELT IN BACK.
EMPLOYEE (MINER) WAS INVOLVED IN A ROCK TRUCK ACCIDENT RESULTING IN $200 +/- DAMAGE TO TRUCK PROMPTING A DRUG TEST. UPON NOTICE THAT HE WOULD BE REQUIRED TO TAKE A DRUG TEST, HE TOLD US HE INJ URED HIS BACK HELPING TO INSTALL TAIL ROLLERS FOR A CONVEYOR AT OUR WASH PLANT. THE INJURY TO HIS BACK OCCURRED 8/9/02.
MINER WAS HELPING CLEAN AROUND PLANT WHEN DUST FELL INTO FACE. HE WAS WEARING SAFETY GLASSES W/SIDE SHIELDS. DUST GOT INTO RIGHT EYE. HE RUBBEDHIS EYE. SUPERVISOR NOTIFIED ON 8/3/01.
WHILE REPLACING BELT DRIVE ON MOTOR TO FEED BELT TO SCALPING SCREEN, FINGER CAUGHT IN PULLEY WHILE CHANGING BELT. RESULTED IN A FRACTURE TO THE AREA BETWEEN THE KNUCKLE AND 1ST JOINT. SUTURES WERE USED ON A SMALL LACERATION IN THE AREA BETWEEN THE 1ST JOINT AND FINGER TIP.
BROKE FIRST FINGER AND MASHED 1ST AND 2ND FINGER ON LEFT HAND. WAS CAUGHT BETWEEN V BELT AND PULLEY WHILE ATTEMPTING TO REPLACE BELTS.
