Associate was attempting to dispose of an air filter, from a 988 FE Loader, when ee went to lift it into the dumpster ee heard shoulder pop. Upon examination from a doctor it was determined that the injury was a torn rotator cuff requiring surgery to repair.

While laying one end down, employee's finger was pinched between the steel flight and steel chain the flight fits into to pull material through crusher. Finger was cut through glove. Received 8 stitches and returned to work.
Employee was walking with a 50 lb. bag of Anfo and EE's back went out.

EE WAS GUIDING BELTS ONTO A MOTOR AT THE SCREEN HOUSE WHEN HIS FINGER SLIPPED BETWEEN THE BELT AND THE SHEAVE CAUSING LACERATION AND FRACTURE TO HIS LEFT SMALL FINGER.
2 EMPLOYEES WERE PERFORMING CLEAN UP BY PICKING UP SCRAP STEEL & PUTTING IT IN THE LOADER BUCKET SO IT COULD BE MOVED & DISPOSED OF. A PIECE OF STEEL FELL OUT OF BUCKET ONTO EMPLOYEE'S FOOT.
EMPLOYEE COMPLAINED OF ELBOW PAIN ON 10/29/01. WAS REFERRED TO ORTHOPEDIC DOCTOR AND RECEIVED CORTOZONE SHOT ON 11/27/01. RESTRICTED DUTY FOR 3 WEEKS.
THE EMPLOYEE WAS IN THE CRUSHER HOPPER ATTEMPTING TO UNPLUG IT. HE WAS PUTTING A CHAIN AROUND A ROCK WHEN THE ROCK SLIPPED PINCHING HIS RIGHT LITTLE FINGER, FRACTURING THE TIP OF THE FINGER.
