The overloads kicked out on the main hoist while bringing miners to the surface at the end of the shift. The overload was reset causing the hoist to operate normally.

A fuse was blown on the hoist and a bad switch was found. Miners were evacuated from underground. A new switch was installed and the hoist was tested. Miners returned to the underground area to finish the work day.
During a routine non destructive test of the hoist rope at the escape shaft, a guide rope was discovered to be bad. All miners were evacuated from underground using the main hoist. The guide rope was replaced and miners returned to work on their next scheduled shift on 4/30/2013.
The air-compressor on the main hoist that released for the brake was determined to be bleeding past the check valve. New air supply was ran to the drake release and the miners were returned to work.

During a routine check, a guide cable was discovered to be loose. All miners were evacuated using main slope. Maintenance employees repaired the cable. Miners then returned underground.
During a routine inspection the main hoist rope did meet the retirement criteria. All miners were evacuated utilizing the escape shaft. The rope was replaced.

While performing normal activites at the escape shaft, the hoist electrical controls developed a minor electrical problem. Two miners hand scaling inside the escape shaft at the time of the malfunction were brought to the surface. A mine evacuation was initiated at 12:45PM and miners were brought out to the surface. Maintenance repaired controls, hoist back up by 1:00PM.

During a normal inspection of the escape shaft, a loose guide rope was observed. Miners were evacuated from underground until the guide rope was repaired. The problem was corrected by 11:35 am.
During a check, the hoist would not operate utilizing the automatic controls. The hoist could have been operated on hand control, but miners were evacuated until the automatic controls were operational.

During a routine check, a guide cable was discovered to be pulled loose off of a ground plate. All miners were evacuated from the mine. After the guide cable was repaired and operational, miners returned to the mine at 10:25 am.
During a routine inspection of the escape shaft, a damaged guide rope was identified on the mancage escape shaft. Miners were evacuated from underground. The escape shaft hoist was back in service at 4:30 pm.

Employee was walking down slope to bottom of B2 when he slipped and twisted right knee. Doctor's appt on 6-7-07 indicated he recommends a right knee arthroscopy.

During a routine inspection of the escape shaft hoist, it failed to operate properly. Maintenance changed out an electrical timer contact and hoist was tested and operated properly. Repairs too approx. 35 minutes to complete.

A roof fall was discovered in our slope on 9/18/05 at 3:30 pm due to power shortage for the surface area. MSHA was notified this day and was onsite 9/19/05. The mine was not in operation this day and nobody was underground. A contractor is performing the cleanup and resupport to the roof and we plan to be in operation by 9/24/05. The main power cable is being repaired also.
During a routine inspection, the guide rope at the escape shaft was found to have a bad spot. Underground miners were evacuated from the mine immediately and a replacement cage was installed. Miners returned underground on 2/1/05 at approx. 10:15 am.

EE WAS GUIDING BELTS ONTO A MOTOR AT THE SCREEN HOUSE WHEN HIS FINGER SLIPPED BETWEEN THE BELT AND THE SHEAVE CAUSING LACERATION AND FRACTURE TO HIS LEFT SMALL FINGER.

EE WAS ON HIS KNEES MATCHING UP GEARS & AXLE ON JCI LOADER. WHILE GETTING UP FROM THE PAD HIS LEFT KNEE TWISTED.
EE WAS CLEANING WINDOW OF HIS TRUCK. AS HE WAS DISMOUNTING TRUCK HIS FOOT SLIPPED CAUSING HIM TO LAND ON BOTH FEET RESULTING IN BACK PAIN.
BELT 2 STOPPED FOR UNKNOWN REASON, CAUSING FRICTION WHICH SEVERED THE BELT. THIS FRICTION RESULTED IN A FIRE AROUND THE NELSON DRIVE. FIRE WAS EXTINGUISHED WITH FIRE EXTINGUISHERS. MINE BELT W AS SEVERED AND A NEW ONE WILL BE INSTALLED.

EE WAS WALKING FROM WASH HOUSE TO PORTAL WHEN HE SLIPPED AND FELL ONTHE SNOW. PULLING RIGHT HAMSTRING.
ANCHOR BOLT BROKE ON GUIDE ROPES ON ESCAPE SHAFT. EVACUATED MINE AT 8:40 AM. BOLT WAS REPAIRED AND MINERS WENT BACK UNDERGROUND AT 10:40 AM.
BEFORE SHIFT DURING TEST RUN, MANCAR HAD COME OFF TRACK. GRAVEL IN SLOPE CAUSED THIS. SLOPE WAS CLEANED, CLEARED & INSPECTED. BENT BOLT ON BEARING CAP OF MANCAR WAS ALSO REPAIRED.
WHILE TESTING THE ESCAPE HOIST CAGE, THE OVER TRAVEL STOPS WERE NOT STOPPING IN RIGHT LOCATIONMINE WAS EVACUATED AT 11:30 AM. ESCAPE HOIST WAS REPAIRED AND OPERATIONAL BY 2:30 PM SAME DAY.

MINERS DID NOT GO UNDERGROUND ON 9-5-01. PROBLEM WITH GUIDE ROPES ON ESCAPE SHAFT WAS DISCOVERED. INSTALLED BULLET CAR ON 9-6-01. MINERS WENT BACK UNDERGROUND ON 9-6-01. MSHA OF VINCENNES CAME AND INSPECTED BULLET CAR.
AN ELECTRICAL PROBLEM WAS DISCOVERED WITH THE EMERGENCY HOIST. MINERS WERE EVACUATED BY MAIN HOIST. ELECTRICIANS REPAIRED PROBLEM AND MINERS WENT BACK UNDERGROUND. COMMUNICATED WITH MSHA OUT O F VINCENNES ON THIS DAY ABOUT THE PROBLEM.
WHILE LOWERING DOWN THE SHAFT A MAGAZINE IT BECAME LODGED AGAINST THE TOP OF THE SHAFT. MINERS WERE EVACUATED BY WAY OF ESCAPE HOIST. MAGAZINE WAS UNLODGED AND SHAFT WAS CLEAR.
EMPLOYEE HAS BEEN DIAGNOSED WITH CARPAL TUNNEL SYNDROME. EE REPORTED THAT USE OF DRILL CONTROLS STARTED SYMPTOMS. SURGERY ON 6-21-01.
EMPLOYEE COMPLAINED OF BACK PAIN IN JANUARY OF 2001, BUT GOT BETTER. EMPLOYEE WAS DIAGNOSED WITH A HERNIATED DISK IN NOVEMBER AND HAD SURGERY ON 11/12/01.
