Was unscrewing the steel plate under the equipment, 3 bolts were loosened when the plate fell onto the employee's arm. the 4th bolt was rusted and did not hold in place.

Employee was working on the dredge. The dredge roof was open and secure. The hinges on the roof broke and the roof fell, hitting the employee on right elbow. In the fall, the employee punctured the side of his left hand, tearing the skin (small skin avulsion).

EMPLOYEE WAS STRUCK BY A PIECE OF METAL IN NECK AREA WHEN ANOTHER EMPLOYEE WAS REMOVING A BUCKET TOOTH ON A BACKHOE. INJURED EMPLOYEE RECEIVED A LACERATION TO NECK AREA REQUIRING FOUR STITCHES.

THE EMPLOYEE WAS CHANGING THE BAG ON THE PAPER SHREDDER. AS SHE MOVED HER HAND PAST THE STORAGE BOX IT CONTACTED A SHARP EDGE ON THE BOX RESULTING IN A LACERATION TO HER RIGHT LITTLE FINGER.
