While cutting grass with the bush hog, employee felt an insect bite on right hip.

Struck by falling object

Was unscrewing the steel plate under the equipment, 3 bolts were loosened when the plate fell onto the employee's arm. the 4th bolt was rusted and did not hold in place.

Employee was working on the dredge. The dredge roof was open and secure. The hinges on the roof broke and the roof fell, hitting the employee on right elbow. In the fall, the employee punctured the side of his left hand, tearing the skin (small skin avulsion).

Absorption of radiations, caustics, toxic and noxious substances

While fueling equipment, the hose came off the fuel nozzle. Diesel fuel splattered into right eye. Employee was wearing eye protection. Eye wash was immediately used to flush right eye.

Contact with hot objects or substances

While using the torch, the disconnect came off, causing 2nd degree burns to the right hand and first degree burns to the left hand and face.

EE (customer and employee at Littl'fellows trucking in Palmetto) was on his way to the scale house, when he reached for his lighter on the bottom of his truck and drove over a berm. The truck ran into a ditch and rolled over. The driver was not injured.

EE HAD STEPPED OUT OF FRONT END LOADER TO REPOSITION PILE STAKE WHEN A FOREIGN BODY ENTERED INTO LEFT EYE. AT THE TIME OF THE INCIDENT THE AREA WAS DRY AND WIND CONDITIONS WERE GUSTY, CREATING A VERY DUSTY ENVIRONMENT, THE FOREIGN BODY CAUSED ABRASIONS TO THE CORNEA AS A RESULT, THE FOREIGN BODY WAS REMOVED BY AN OPTHAMOLOGIST

THE EMPLOYEE WAS CHANGING THE BAG ON THE PAPER SHREDDER. AS SHE MOVED HER HAND PAST THE STORAGE BOX IT CONTACTED A SHARP EDGE ON THE BOX RESULTING IN A LACERATION TO HER RIGHT LITTLE FINGER.

THE EMPLOYEE WAS OPERATING LOADER AND WAS TRAVELING TO THE SHELL PLANT. WHILE TRAVELING THE MACHINE STRUCK A BUMP ON THE ROADWAY CAUSING A STRAIN TO LOWER BACK. EMPLOYEE REQUESTED TO SEE A DOC TOR ON 6-24-03.
