Employee had machine properly locked out. EE was removing a cover to clear a jam in the machine. EE dropped the cover on EE's right middle finger causing fracture.

originally Diagnosed with a non reportable strain to EE's left knee, sustained while connecting a hose to a tanker truck. On follow up with Orthopedic Dr. 02/12/26 EE was scheduled for MRI and PT. Case elevated when EE started medical treatment (physical therapy) 03/09/2026.

Employee was initially injured on 7/8 at 0830am as EE tweaked EE's shoulder. This was initially treated as a first aid for a strained left shoulder with no work restrictions. Based on radiation of symptoms to EE's right upper extremity and lack of pain improvement, an MRI was ordered which resulted in the reclassification from first aid to recordable was made on 8/6.

Employee was attempting to lift elevator door on man elevator and door stuck which caused EE to strain EE's back. Was still working at full duty, became a recordable on August 20, 2025. Prescribed medications and also limitations. Able to work with limitations.
Driller was in the process of drilling boreholes, dust hose collection was not operating properly. Upon inspection of the dust collection hose, driller saw an obstruction. EE removed the hose to clear debris from the hose and upon re-attaching the dust collector hose, EE's right hand came in contact with the metal fastener on the hose, cutting EE's right hand.

EE was going up to the top of the upper shelf lime silo to unclog and get it to flow when EE opened the smaller round door to probe the lime or rodding if you will, it broke loose and came out of there like water getting all over EE and into EE's right eye.

first aid incident occurred on 10/30/23. No medical treatment prescribed. After months of continued pain, employee asked to be seen by doctor again. Doctor prescribed medical treatment (physical therapy,) elevating the first aid case to a reportable accident.
EE was helping change motor on sand line palletizer. The crew went to tighten the motor but had an alignment issues with the motor. When they tried to adjust the motor it started to fall to the ground. EE took the motor slowly to the ground and in the process bruised left forearm.

Employee was unloading bags from the reject shoot (roller bed that sends bag down to table, for removal by hand.) The employee lifted a bag, then turned to put bag on a pallet. When EE turned, EE struck EE's knee on the leg of the table. Dislocation of right knee cap.

Employee was assisting in the installation of a drive unit on a mixing screw. The unit was under tension, being pulled into alignment. Employee was using fingertip to check alignment of bolt holes. The unit unexpectedly and suddenly moved, causing the amputation of left pinky fingertip including the fingernail.

Large grains of sand fell into left ear. Miner was unable to remove sand, and was sent to clinic to have ear irrigated. Two large grains of sand were removed, and antibiotic drops were prescribed to prevent infection.

Operator was handing bulk bags, when removing bags from hooks EE got hand caught between strap and bag. Was not following proper communication with forklift driver and other operator.

Sharp edge of banding on bundle of burlap bags cut employee's hand, while employee was moving the bundle. Laceration to left palm required 5 stitches. Employee was returned to work with limited use of hand for 10 days. Follow up to be done by attending clinic, in 10 days.

EE was washing under the feed hopper, a baseball sized rock dislodged, striking EE's hardhat causing a cut on EE's head. Treatment was offered and refused, refusal waiver was signed. After work, EE sought treatment, receiving one staple in EE's head. No documentation was received. EE was on vacation for the next three days.

On 5-2-14 employee and a co worker were changing screens on the mill. When they were lifting off the cover plate EE felt a "pop" in her right wrist. I asked if she needed medical care. She refused. On 5-7-14 EE came into the office and said her wrist was sore and she would like to get it looked at just in case. I sent her to HRRMC in Salida Co.

Employee was pulling material out of the Dust Collector for the Q-Line from the opening above him. A chunk of material fell from inside the container and fell onto his head/face. Employee was not wearing eye protection as required by company SOPs. Portland cement dust got into his right eye.
Employee claims to have dropped a empty pallet on foot. Employee worked from 3:30 until 4:30 and did not report the accident. Employee reported accident following morning via voice mail. Accident resulted in a fractured toe. Employee missed one day of work and returned with no restrictions

The employee climbed up the front of the shaker screen where there is no safe access and was cleaning excess material off of the screen chute. When he finished he was climbing down and snagged his glove on the frame of the concrete conveyor causing him to over extend his arm.
