originally Diagnosed with a non reportable strain to EE's left knee, sustained while connecting a hose to a tanker truck. On follow up with Orthopedic Dr. 02/12/26 EE was scheduled for MRI and PT. Case elevated when EE started medical treatment (physical therapy) 03/09/2026.

Worker was placing bag on fill spout and put their finger under the bag clamp. The clamp crushed the tip of their right ring finger causing an open fracture of the tuft of distal phalanx of the finger.

first aid incident occurred on 10/30/23. No medical treatment prescribed. After months of continued pain, employee asked to be seen by doctor again. Doctor prescribed medical treatment (physical therapy,) elevating the first aid case to a reportable accident.

The employee used a piece of scrap floor grating in an attempt to dislodge material blocking the discharge chute of the dryer bag house. The metal twisted in the employee's hand, causing a laceration of the right index finger.

Employee was unloading bags from the reject shoot (roller bed that sends bag down to table, for removal by hand.) The employee lifted a bag, then turned to put bag on a pallet. When EE turned, EE struck EE's knee on the leg of the table. Dislocation of right knee cap.

Employee was assisting in the installation of a drive unit on a mixing screw. The unit was under tension, being pulled into alignment. Employee was using fingertip to check alignment of bolt holes. The unit unexpectedly and suddenly moved, causing the amputation of left pinky fingertip including the fingernail.

Employee reported that the employee pulled a muscle in employee's back while moving a 50# bag of material

The minor had an undisclosed, preexisting medical condition. EE went into a seizure while at work. When ee seized, ee fell and struck head on the floor, resulting in a minor laceration to scalp. No equipment or machinery was involved

Struck against a moving object

Employee ran into pot hole while driving forklift, resulting in cervical strain. Employee returned to full duty for full shift on the following day.

Sharp edge of banding on bundle of burlap bags cut employee's hand, while employee was moving the bundle. Laceration to left palm required 5 stitches. Employee was returned to work with limited use of hand for 10 days. Follow up to be done by attending clinic, in 10 days.

Employee was climbing down from a tractor (truck) cab and stepped on uneven ground. EE's ankle turned under, causing a sprained left ankle and a fracture in the left fibular.

Employee was attempting to dump portable dumpster when his hand slipped cutting his wrist on the lever.

Employee went into breakroom to get a bite to eat and take a break. Employee was found unconscious in the breakroom by another employee on site approximately 15 to 20 minutes later. Determination and cause has not been determined at this time.

Employee was pulling material out of the Dust Collector for the Q-Line from the opening above him. A chunk of material fell from inside the container and fell onto his head/face. Employee was not wearing eye protection as required by company SOPs. Portland cement dust got into his right eye.

Employee was picking up 50# AP sand bags and went to put it on a pallet when he felt his shoulder R pop out and in and now he is having pain in his shoulder. Diagnosis: Right Shoulder Stain.

Employee was on the sand line packer, a bag got caught he jumped down to clear the bag from the belt and hit his knee on a piece of steel, left knee.
