Just finished up a blend and were cleaning off a grate on the blender. 1 man was holding the grate up as IE was wiping their hand under the grate on the flat part of the blender, cleaning the excess powder. The man holding the grate didn't know the hand was under the grate and dropped it on the tip of IE's right hand middle finger tip.

Picked up bearing housing from back of welding truck. Brought it through door of shop, lifted it up to table and felt something pull and burn beneath rib cage. Housing weighs 45-50 lbs (as per employee's description).

EE WAS WORKING ON RETURN PUMP AT WASHER PLANT WHEN HE FELT SOMETHING PULL IN HIS GROIN AREA AND DISCOMFORT BEGAN. NOTE: AS OF THIS DATE HE HAS NO LOST TIME. HOWEVER, THE DR. STATED THAT HIS HERNIA WILL HAVE TO BE REPAIRED AND HE IS REFERRING HIM TO A SURGEON.
EE WAS WORKING ON PACKER AND AS HE TWISTED AND LIFTED, HIS MUSCLES TIGHTENED CAUSING LOWER BACK PAIN.
EE TURNED TO CATCH A LADDER BEFORE IT FELL AND TWISTED HIS BACK.
EE WAS CLIMBING THE LADDER TO GET IN HIS TRUCK, APPROX. 8 FT. TO GOT TO A STEP AND HIS BACK STARTED HURTING. HE THOUGHT AND WOULD BE OK. HE DIDNT THINK HE NEEDED TO GO TO THE DR. AT 1:30 PM HE TOLD OTHER THAT IT WAS STILL HURTING. OTHER RETURNED TO THE OFFICE AND CALLED COMP FIRST 2:25 PM HE THEN CALLED UPPERT CARE AND SENT HIM TO THE DR.
