Struck against stationary object

Sharping a machete in a vice. Employee had a hand held sharpener and was pulling it across the blade. Employee's hand slipped off the sharpener handle and the right bottom ring and little fingers hit the blade.

Just finished up a blend and were cleaning off a grate on the blender. 1 man was holding the grate up as IE was wiping their hand under the grate on the flat part of the blender, cleaning the excess powder. The man holding the grate didn't know the hand was under the grate and dropped it on the tip of IE's right hand middle finger tip.

Employee was performing maintenance on palletizer, making adjustments to turntable. Employee pulled on wrench to loosen a bolt on the bearing & felt a pinching pain in neck & shoulder; reported incident to supervisor on 05/30/17 because pain was still present. Employee was scheduled off work on 05/31/17 & went to clinic for checkup (no time was lost from work).

Employee was cleaning iron oxide (umber) from inside the bottom cone of the bag house; inserted right hand into enclosed chute leading to moving air lock; fingertip of right index finger was partially amputated.

Accident type, without injuries

Employee was driving dump truck loaded with gravel to repair muddy road leading to mine site. Road was wet and muddy, employee drove through soft area on road and truck partially tilted over on driver's side.

Employee was walking up stairs; toe of boot caught on grate and caused employee to fall, hitting left arm/elbow on toe board. Employee declined medical attention at time of incident, but sent for medical attention at 2:30pm on 10/28/2015; advised to wear sling on left arm and not use arm for 2-3 days. Employee was not scheduled to work again until 11/02/2015.

Employee was picking barite on picker belt; large rock rolled back down the conveyor belt that had already passed employee and rolled on top of left hand and four fingers.

Employee was patching a hole in a water pipe. He stepped across/stood over pipe to weld patch in pipe; right foot slipped and twisted right ankle.

Employee was changing screens at Washer and screen frame fell on right index finger causing bruising and soreness.

Picked up bearing housing from back of welding truck. Brought it through door of shop, lifted it up to table and felt something pull and burn beneath rib cage. Housing weighs 45-50 lbs (as per employee's description).

Fall from machine

Employee was checking oil and greasing WA600 loader. Stepped down and missed the step and fell to the ground, hurting lower left side of back, left hip and left leg. Employee thought he would be okay, so he did not go to the doctor at the time of the incident. On 11/27/2012, employee was still in pain and was sent to the doctor for evaluation.

A 22 ton rough terrain crane was traveling up an incline on a dirt access road when the crane stalled, lost power and steering, rolled back into the berm on the dirt road, turning over the crane and trapping the operator inside.

Employee was walking across catwalk to his workstation at the water gun. He slipped and fell into the guardrail and mashed his hand on the guardrail with his body weight. He jammed his left index and middle finger.

Employee went to adjust gap between top roll and end of conveyor chain. Co-worker started chain conveyor. Injured's left little finger hand was pulled into gap. The conveyor stopped. Other employee came to victim and tried to pull his hand out but could not. He then got a steel bar to widen gap and pulled victim's hand free.

THE LOADING SYSTEM KICKED OFF AT SEPARATOR PLANT AND THE EE WENT TO TRIP SWITCH WHEN HE TRIPPED OR SLIPPED ON STEP. HE FELL AND CAUGHT HIMSELF WITH LEFT ARM IN AN AWKWARD POSITION, DISLOCATING HIS SHOULDER.

EE MOVING MUDBALLS W/LOADER; HEARD SOMEONE FALLING DOWN CHUTE, REVERSED TO BACK OUT OF WAY. THE PERSON HIT THE LIP OF BUCKET, BOUNCED UP AND HIT THE TOPS THEN CRASHED THRU THE WINDSHIELD. GLAS S BROKE LENS OF GLASSES ALLOWING FOEIGN MATTER THE LEFT EYE.

AT 1:30 PM HE TOLD OTHER THAT IT WAS STILL HURTING. OTHER RETURNED TO THE OFFICE AND CALLED COMP FIRST 2:25 PM HE THEN CALLED UPPERT CARE AND SENT HIM TO THE DR.

EE WAS IN THE MAINTENANCE SHOP HELPING TWO OTHEREMPLOYEES WORK ON THE CYLINDER OF A LOADER. THEYWERE TRYING TO TURN A 4"" NUT ON THE CYLINDER USING A 4"" PIPE ON THE END OF THE WRENCH TO HELP TU RN THE NUT. THE NUT CAME FREE AND THE PIPE FELL OFF OF THE WRENCH AND HIT TOP OF INJURED'S FOOT.

HE WAS SITTING ON THE LOADER THAT HE WORKS ON. AS HE WAS SITTING HE SAID HE FELT A TWINGE IN HIS BACK. HE TOLD COWORKER THAT HE NEEDED TO THE DR. SENT TO MEDICAL CENTER. TOLD THE RETURN TO REG ULAR DUTY. HAS FOLLOWUP APPTS. FOR THERAPY. THIS WAS A REOCCURANCE OF AN INITIAL INJURY.
