Employee was lifting multiple 5 gal buckets of rocks up to a makeshift chute for clean up. They started to feel lower back pain but continued thinking it was only a muscle issue. They took the next day off day due to pain and returned the next day and reported it. They aggravated a previous injury and inflamed the area, the Dr placed them on light duty for 5 days until review

Employee was attempting to move an airslide in Silo 2 to clean it out when they felt pain in groin area. They reported it that day but continued work, the next day they felt worse and was taken in to get it checked out. The ER determined a partial muscle tear in the groin area. Employee has been placed on light duty and unable to lift more than 10lbs until cleared by a Dr.

A quarry truck driver sustained a left index finger pinch injury while manually moving a ~288 lb motor that was left obstructing the Secondary Crusher Hoist way walkway. With the hoist out of service, a pony bar was inserted in the lifting eye so two employees could lift it onto a pallet. During the lift, the motor shifted and slid down the bar, pinching the employee's finger.

The employee removed the calibration weight and it dropped onto the employee's hand, pinching it between the top of the guard and the weight. As a result, the employee sustained a cut that required 8 stitches and a hairline fracture to left hand ring finger.

Employee lifted 80-170 lb liners for mill #3 outage and developed right-hand pain after a popping sensation. On May 8 the tendon tightened again; diagnosed with trigger finger. Surgery occurred 1/8/26, with no use of the right hand until 3/11/26. Employee has returned to work with restrictions on 2/2/2026.

Description of Incident: Employee was attempting to remove pallet from lowered hi-lo forks by hand and sustained a small laceration of left middle finger from an exposed packing nail. Employee was taken to ER and treatment was rendered in the form of two stiches. Employee was cleared and returned to work.

The employee, wearing glasses and a face shield, was in the process of removing the kiln shelf feed door for inspection when dust unexpectedly puffed into both eyes from the opening on the feed shelf. The employee was prescribed eye drops on march 4th, 2024.

On September 25th, 2023, at 1:00 PM, an incident occurred at Finish Mill #3 during a repair operation on the water lance. The employee sustained an injury to EE's right ring finger when EE accidentally pinched it between a pipe EE was carrying and a handrail. The injury was assessed as requiring medical attention and received two stitches.

The employee was installing new buckets on FM#3 discharge elevator. The bucket weighted roughly 25lbs and was lifting the bucket at chest level. Employee has been doing physical therapy from 5/25/2023 until 1/31/24. The doctors diagnosed EE with a torn rotator cuff and went into surgery 1/31/24.

Employee was lifting cribbing to set the mill motor in place on a flat bed trailer. When the employee was lifting the cribbing (approximately 100lbs) employee felt knee hyperextended backward resulting in pain. Employee was seen on January 6, 2023 by the orthopedics and was given a cortisone shot.

The #3 boom boat loading sock counterweight was stuck in the up position and needed to be freed up. The employee decided to pull on the cable to free the counterweight. When the counterweight broke free it pulled employee's hand towards the pulley. This resulted in cutting the tip of the pinky off.

The employee was instructed to go up to the 316 belt and clear debris. The employee proceeded to the platform to perform the function. The employee went to place the shovel on the platform. While putting the shovel up on the platform, the shovel made contact with the hand rail and bounced back with the flat part of the metal shovel pan hitting the employee in the nose.

Miner entered a confined space that shut down 8 hours before hand. The miners boots were not tied. 6" of hot material was at the bottom. The miner stepped into hot material. That material got into EE's boot and burned EE's foot. 2nd degree burn.

The employee was carrying a 170lb coupler up 5 flights of stairs in the PH tower on rt. shoulder to repair the kiln feed elevator. The employee reported a shoulder strain injury and received first aid on 12/20/20. Employee requested on 1/19/21 to be evaluated by medical professional and placed on restrictions. Employee received surgery on 6/30/21 and experienced lost time.

A miner with almost 14 years experience was helping reattach expanded metal to guard framing at #2 Kiln Pier when a piece of sensory flat iron rotated into the work area and hit EE's forehead. The miner continued to work for 2 hours before requesting treatment, was then taken to the hospital, treated and brought home.

EE was cleaning area for compressor to be installed and was struck by PVC pipe. EE said EE felt fine but wanted to get checked out. EE is on FMLA and usually takes 1-2 days off a week. EE was taken to emergency and was released without any restrictions. EE went to EE's FMLA doctor and asked for medication for the tightness and was given rest of week off.

Miner was greasing a finish mill bearing with a portable 3 wheeled greaser. The hose did not reach by 6 inches so the miner tried propping the greaser on a block of wood. While lifting the greaser the front caster wheel spun and pinched miner's finger causing a cut to left finger. The cut was serious enough it resulted in restricted job duties until healed, no stitches.

Cement dust that had built up along a wire tray. EE went to pull wire along the tray, it knocked the material out, causing it to spill down on EE's head & face. EE was wearing hard hat/safety glasses. Debris was flushed out but there was still residual that had to be removed by a Dr in the L eye. Pain & anti infection medication was prescribed. EE was released w/o restriction.

Shift started at 6pm prior evening. During the working shift the employee was clearing raw kiln feed from a transport drag by vacuum method. The hot feed was being cooled by water application to prevent vacuum hose from collapsing. The hot material sloughed into pooled water creating a steam rush that burnt the miners legs. Incident investigated by mine inspector.

EE felt pain in right shoulder while traveling mortar product overhead in a newly constructed cyclone plenum. EE didn't seek medical treatment believing it was just a sore shoulder. EE returned to Wear Con facility on 5/8/17 where EE sought medical attention and was diagnosed with a right shoulder muscle and tendon strain of the rotator cuff and place on restrictive duty.

Mobile mechanic was reaching in changing a hose on a 992 CAT loader. EE felt a pain in the side. EE reported the pain and refused treatment as to the assumption it was a strain. On 4/18 EE told doctor about the pain and an x-ray showed EE had fractured ribs. Note: employee is being evaluated for another condition possibly contributing to injury. No missed work.

Employee was setting horizontal rebar (size 11) and to help maneuver it, grabbed the end of the rebar with right hand. When sliding the bar, right index finger was caught between the horizontal bar being placed and a previously placed vertical bar, causing a small fracture to the distal phalanx and a laceration, which required sutures.

Contract laborer was cleaning up a gypsum rock spill on grating when EE was struck by a piece of gypsum. A single gypsum rock fell from a beam just as the employee bent over to pick up a piece of material and fell between the adjustment strap and brim of EE's hard hat resulting in a small contusion that required a stitch to close. The employee was given full return to work status.

EE, a Thermico, Inc. was working as a contractor at the St. Mary's Cement plant in Charlevoix, MI and the clamps holding a 4-ft bar broke loose and fell 40 feet striking the employee in the right upper arm. He sufferer a fracture of his right upper arm and was treated at the local hospital.

Working on the #3 discharge elevator. Lifted a bucket that was stuck. Felt a pull in the back. Ignored it. Kept working. He is now feeling symptoms in the left leg and went to the dr. yesterday. He has been placed on a 10 lb lifting restriction and anti-inflammatory prescription. Seeing doctor next week.
