While doing preshift duties, ee stepped in a hole. Later in the shift while shoveling coal, he felt a pain in his waist running down his leg. Pain was minor but noticeable. Did not miss any work until April 8th when pain became more intense. He went to doctor who advised an MRI and time off.
Injured was installing a yellow rope cable hanger. Injured jumped up & hooked the hanger hook over the roof bolt plate & when he came down he caught the last 3 fingers of his right hand in a loop of rope tearing skin off end of pinky finger & bruising the other two fingers. The man was left hanging by his fingers & coworker had to lift him up to get fingers loose.
Injured had pushed supplies off the scoop with the ram. He had a chain hooked around the supplies & anchored to a hook on the ram. The chain was too tight to unhook & he was holding on to the chain with his right hand & he stepped onto the chain attempting to get slack & he caught his finger between the chain & the hook cutting his finger & causing a distal fracture.
EE was talking to supervisor and while bent over due to seam height EE's hard hat scrapped the mine roof causing rock dust to fall behind his safety glasses. The rock dust struck him in the eye. EE has not missed any days worked~medical treatment onlu using prescription mediciation for treatment of the material in the eyes.
EMPLOYEE WAS HELPING PUT IN BELT STRUCTURE. A COWORKER HAD PUT ONE RAIL IN THE STAND. INJURED WAS TRYING TO PUT HIS IN. COWORKER THOUGHT INJURED HAD HIS IN AND HIT THE FIRST RAIL, THIS KNOCKED INJUREDS OUT, IT FELL AND SMASHED HIS FINGER
EE WAS CRAWLING ACROSS CONTINUOUS HAULAGE SYSTEM (BRIDGE) AND STRUCK HIS KNEE. EE CONTINUED TO WORK AND DID NOT RECEIVE ANY MEDICAL TREATMENT UNTIL 10/21/04; AT THIS TIME THE OCCURRENCE BECAME REPORTABLE.
Caught fingers of right hand between boom and stab jack of roof bolter while attempting to clean dust hose and another operator moved roof bolter head.
EE WAS LANDING AN IDLER ROLLER ACROSS A TRUSS TO A SECOND EE FOR INSTALLATION AND FELT A SHARP PAIN IN HIS BACK. HE CLAIMS THAT HE HAD HAD REOCURRING PAIN SINCE THAT TIME AND DECIDED TO HAVE IT CHECKED OUT ON 9-12-03 AND THE SAFETY DEPT. BECAME AWARE OF THE DIAGNOSES ON 9-14-03
