While moving a rock out of the feeded the rock slipped from his hand causing a cut.

While hauling water line employee's finger was mashed

On the first right panel 002 section #6 belt in the #4 entry a rock fall measuring 1'x17' fell affecting ventilation

At break 14 #1 belt a rock fall occurred measuring 6'x2' x8"" which knocked a hole in the stopping.

A roof fall was discovered between the #3 & #4 entry at break 23

Illness

Employee was finishing up the right side of his cut while loading a buggy when he felt something hit him in the leg causing a laceration to his lower left leg.

Roof fall at # 17b break in the neutral air course.

A mechanic was removing an old U-joint from a coal truck. In the process of striking the object to punch the U-joint out, the mechanic delivered a glancing blow, which caused a 6 inch long x 5/8 inch thick bolt to fly out and hit the injured party in the mouth. This caused a cut to the mouth, one tooth to be knocked out, one tooth broken & two teeth to be loosened.

A rock fall occurred above anchorage at crosscut 14 between #2 and #3 return entries along the #1 belt at spad #176 The fall knocked out a return stopping. No one was injured or trapped, no damage to equipment. The return air course was rerouted.

An area near a previous roof fall had been cribbed the rock shifted causing the cribbing to fall. No damage was done and travel and ventilation was not affected.

The area of the fall had been bolted with 4' glue bolts, a rider seam 5' to 6' high let loose allowing a section of roof to fall. Several cribs were set in the area.

An unplanned roof fall above the installed roof bolts. The fall is located at #18 cross cut along the #1 belt between #2 and #3 entrys. No one was injured, trapped and no equipment was involved. The fall was an extension of a fall that was reported 6/22/09.

Roof fall above ancorage occurred in the primary escape way at break 11 along #1 belt at spad #161. The fall was 6 to 7 feet high 19' wide and 22' long. No injuries, no equimpment involved, and ventilation was not impaired.

Employee was coming from deep mine fully loaded; he was speeding, he was in 7th gear, he missed the curve flipping over the truck.

An unplanned roof fall above the ancorage zone of the installed roof bolts. The fall is located at #18 belt (did not fall on belt) between 2 and 3 entry. The fall knocked a return stopping. No one injured or trapped, no damage to equipment.

Putting up 8 ft bolts when bolt got hung up on drill steel pinchers. The up pressure was hit in an attempt to get the plate unhung. Holding on to the plate, the bolt bent, pinching his finger against the drill steel pincher; requiring 12 stitches.

While doing preshift duties, ee stepped in a hole. Later in the shift while shoveling coal, he felt a pain in his waist running down his leg. Pain was minor but noticeable. Did not miss any work until April 8th when pain became more intense. He went to doctor who advised an MRI and time off.

A roof fall occurred at crosscut 19 between #5 and #6 entries. The fall measured 5'H x 20'L x 19'W. There were no injuries but a stopping was destroyed interupting ventilation. The area had been bolted with 4' bolts.

Injured was installing a yellow rope cable hanger. Injured jumped up & hooked the hanger hook over the roof bolt plate & when he came down he caught the last 3 fingers of his right hand in a loop of rope tearing skin off end of pinky finger & bruising the other two fingers. The man was left hanging by his fingers & coworker had to lift him up to get fingers loose.

EE & other were running water thru the spray plaster machine to flush it out & the nozzle plugged up. When the nozzle cleared itself, the hose jumped out of work hand & sprayed him in the face. The force knocked his glasses off & got material in both eyes. Transported to hospital,had eyes flushed & given antibiotic.

Injured and co-worker were attempting to clear a rock from under the foot jack of the TRS. Co-worker was prying on rock with a fiberglass slabber bar and injured was trying to move the rock with his hands. The bar broke and the injured's middle finger on his left hand was caught between the rock and the foot jack causing a fracture.

Injured and co worker were walking thu the crosscut from #4 to #3 entry. Employee stated he was looking at the roof and he stepped into an offset in the bottom causing him to fall to the bottom spraining his right knee.

A roof fall was found in #2 entry at crosscut #19 near spad #206. The fall measured 20'X20'X4-5'high. No injuries or equipment damaged as a result of the fall. Area had been bolted with 4' resin bolts.

Employee stated he was stuffing rags into air conditioner when his finger hit the fan.

A roof fall was found in #7 entry on No 1 Mains between crosscut 21 and 22. There were no injuries or equipment damage. The fall measured 40'L x 20'W x 5'H. The area had been bolted with 4' resin bolts.

Injured and co-worker were rehanging line curtain in #3 entry when injured bent down to pick up the curtain, a piece of rock measuring 3 1/2' x 5' x 2"" fell from between the roof bolts striking injured in the back between the shoulder blades causing hairline fractures of 3 vertebrae. Injured was treated and released from the hospital.

A roof fall accurred in the #7 entry on the mains inby spad #189. There were no injuries or equipment damaged. The fall measured 19'Wx50'Lx4-6'H.

Employee was attempting to straighten a bent 8' bolt. The end of the bolt was in the hole and when he shoved on the bolt he caught the end of the ring finger between head of bolt and drill chuck causing a fracture.

A roof fall was found at 11:00 AM in the mains in #7 entry at crosscut #19. There was no equipment damage or injuries. Fall measured 25'L x 18'W x 4 1/2'H. Area had been bolted with 48"" resin bolts.

Roof fall occurred in intersection of #1 entry and crosscut #21 in the return. Fall measured 20' W x 30' L x 4' H. Area had been bolted with 4' resin bolts. No injuries or equipment damaged.

Roof fall occurred in the intersection of #6 entry and crosscut #12 in the mains. Fall measured 28' L x 22' W x 4' H. Area had been bolted with 4' resin bolts. No injuries or equip. damage.

Mine examiner found roof fall at break #22 in #1 entry on the mains. While doing his preshift. Fall measured 35'l x 19'w x 5'h. Area had been bolted with 4' resin bolts. No injuries or equipment damamge.

Roof fall occurred outby in face of #5 room at break #19. The fall measured 30'L X 20'W X 4 1/2'H. The area had been bolted with 4' resin botls. No injuries or equipment damaged.

Roof fall occurred in intersection of #1 entry and crosscut #13. Material fell out to the slicken side top just above the anchorage. Fall measured 38'L x 18'W x 4'H. Area had been bolted with 4' resin bolts. No injuries or equipment damage.

Roof fall occurred in #2 entry at intersection at crosscut #17. Material fell out to slicken side above the anchorage. Fall measures 30'L x 35'W x 5'H. Area was bolted with 4' resin bolts.

Employee was spraying stopping with fosrock when hose blowed off end of sprayer striking employee in the face resulting in material going into both eyes. Medical reportable only employee was released (material flushed from eyes and drops put into eyes)

Roof fall occurred #5 Entry (Neutral) between #14 -#15 crosscuts. Fall was approximately 72'L x 19'W x 3.5' - 6'H. No injuries or entrapment, no ventilation or passage affected, no equipment damaged. Fall was found on preshift at 10:35 pm.

Roof fall occurred at crosscut 22 inby the feeder on the belt entry. The fall was 65' long x 20' wide x 6' high. The area had been bolted with 4' and 6' resin bolts.

Roof Fal occurred on mainline belt entry between crosscut 17 & 18. Fall measured 4'H x 34'L x 20'W fall occurred at anchorage point. Was bolted with 4' resin bolts.
