Picked up a box of glue for the roof bolter and strained his back the employee started missing work on 11/5/2013

Pulling cable off of bolter hurt lower back worked Tuesday and starting missing work on Wednesday.

Mounting and dismounting tires when he was picking up tire felt pain in lower back took a 15 minute break then continued working the rest of his shift with a little pain went home and wife checked his back it was swollen same place as before went to the ER in Louisa KY said he had lower lumbar strain.

Was building a brattice bent over to pick up a block felt pain in lower back and could not straighten up.

Opening a bucket of plaster splashed out and got in his eyes. Employee did not start missing work till 6/13/2013 and returned to work on 6/15/2013.

Getting pump car ready to move had finger in the top of door and door shut on finger

Picked up roof bolts out of scoop bucket turned towards roof bolter and legs gave out hurt his back.

Changing a wet brake system on a scoop, picked the wet brake up, felt pain in lower back and legs. Started missing on 7/23/2013

Employee was stacking block to build stopping and bent over to pick up block he turned while lifting and felt a sharp pain in lower back and left hip.

While installing a clutch on a truck, the clutch started to fall, tried to hold it up and injured his back. Continued to work started missing work on 2/25/13.

Setting oil cans on back of rubber tire ride and twisted his back.

While shoveling around the No.3 tailpiece a piece of coal bounced off the belt, striking him on the forehead.

EE stated stated while lifting a part from the floor, he injured his back.

Installing a front idler on a drill, had boom controller in right hand and slate bar in left hand. He was reaching the slate bar to another mechanic who was under the drill. Felt a burning sensation in his upper back.

EE stated that while climbing the ladder to get in his truck, he felt pain in his right shoulder.

EE stated that while getting a knot out of the miner cable, it fell from the hanger and struck him on the head and right shoulder.

Employee stated that while shoveling coal from under the belt, he twisted his back while he shoveled the coal away from him.

Employee stated that while lifting a 5 gallon bucket of material, she felt pain in her arm and shoulder.

Employee stated that when he threw a chain across a set of batteries to secure them on a truck, he felt a sharp pain in his back.

EE stated that while mounting the load grader, he twisted his knee.

Laying track lifted rail & strain neck & lower back. Did not stop work until 6/1/07. Reported injury to companyon 6/6/07.

Employee stated that while taking the radiator cap off the generator, steam came from the radiator causing burns to the right arm.

While changing filters on a 992 loader, he had to pound on the filter to loosen it, injuring his right arm.

Changing track pads on drill tracks. While lifting pads & installing on new rails, pulled muscle in stomach.

Working on #4 belt head bearing, cleaning off, cut right thumb on shaft; had to have 6 stitches.

Changing hose on scoop. When fitting broke loose twisting body causing pain to rib cage.

Employee stated that while loading an oxygen tank into the cage, he felt a sharp pain in his lower back.
