While driving along haul road a piece of gravel flew from tire, hit the mirror and busted the glass. A piece of glass hit the driver in the eye.
Driver stated while going thru the roller coaster dip the transmission started making a clicking noise. So he down shifted and the truck rolled backward into the ditch and caused the truck to turn over on the passenger side.
Employee stated that while dismounting a D10 dozer he slipped and twisted his knee and ankle.
Employee using air sander to clean blade on grader. Sanding disk came apart, piece struck employee in the stomach.
Driver stated that as he came into curve truck started to turn and slide coming into and while trying, to go around the curve like the Jake Brake was shoving him as it kicked in.
