Employee stated that he was loading a 5 gallon hydraulic oil can on a pallet and he twisted his back. He said he felt a pop in his lower back. Employee went to the doctor over the weekend and was diagnosed with a pulled muscle. He notified the mine site on 9-1-15 that he would be on light duty for 1 week. Employee returned to regular duty on 9-8-15

Employee stated that he was cleaning his permissible ride and attempting to water the batteries. The ride has a spare tire attached to the battery lid cover and when he moved the spare tire, he felt a muscle strain in his back. The employee finished his shift but he went to the doctor on 3-15-15. He was placed on light duty for 1 week with a return to full duty on 3-23-15.

Employee & foreman were training conveyor belt and employee lifted the structure to lengthen the chain that the structure was suspended from the roof with. Employee jerked up on the belt rail and then felt a pain in his lower back. employee was given restricted duty. Employee was working as a mine examiner.

Employee was attempting to raise the flip up shield on the continuous miner and as he was lifting the shield his foot slipped causing him to let go of the shield in order to catch himself. This resulted in the shield coming down and mashing the employer's hand. The employee received a crack to the 3rd metacarpal bone of the hand.

Employee states that he was helping to move a water pump. After moving pump he felt a burning pain in his knee. Employee informed company on 11/12/2013 that he has been going to physical therapy.

Employee states that he had lifted a rub rail to install on the miner. After lifting employee states he felt tightness in his back. Employee states that he then went to remove the rub rail and felt pain in his back. Received documentation on 9/17/2013 that employee was diagnosed with a lumbar strain.

Employees states that he bent over and picked up the end of a 6 inch Aquamine pipe and then turned to walk and felt pain in his back. Employee did not report incident to supervisor. He reported the event on 8/2/2013 @ 8:21 PM on the call in line. On 8/6/2023 employee called and asked if he needed to come in and have an incident report filled out.

Employee and co-worker were installing track rails and had positioned a mine jack under the rail and had lifted the rail off the mine floor in order to install a rail tie. Employee asked the co-worker to use a bar and pry on the rail. When the rail moved the rail fell from the jack and struck employee on the foot. This resulted in a fracture to the foot.

Employee received a laceration to his left forearm while helping bolt crew load their bolts onto roof bolter. The steel band that was binding the bolts together had been cut and 200 of the 250 bolts had been loaded. The steel band suddenly sprung and struck employee in the arm resulting in a laceration.

Employee stated he was pulling a timber out of a banded pile of timber and had to pull hard and when the timber suddenly became free and caught his finger between the timber and the scoop bucket. Employee was seen at local hospital & released to work with no restrictions. Received medical documentation on 5/11/2012 that his finger was fractured.

Employee was loading an empty wooden pallet onto an empty rail supply car and he felt a sharp pain in his arm and shoulder. Employee continued to work until 7/28/2011. Company received medical documentation on 9/28/2011 that incident was reportable.

Employee was using a crib block to bump a motor in place. The old motor had not been removed from the work area and as employee swung the crib block back,he struck the old motor with his left hand. This injury resulated in a laceration & slight fracture of the ring finger of his left hand. One suture was required to close wound

Injured was in the process of opening the first set of airlock doors at the mine portal, when he got his right hand pinched between the doors. This caught between injury, resulted in a fracture to his right ring finger.

While making a belt move, the crew was positioning the tailpiece and applying tension with the scoop to align the belt. A chain broke that was hooking the scoop to the tailpiece, which allowed the tailpiece to slide around and strik the employees foot, resulting in a fracture to his foot.

Miner was removing a piece of rock from the left side continuous miner on the 1st right section. The rock broke causing his right hand index & middle fingers to come in contact with the sharp edges of the rock causing a laceration that required 9 sutures.

Employee was removing a nylon cable hanger from a miner power cable and water line. While removing fron a roof bolt the cable got tension on it and as the hanger was removed, the s-hook fastener cut employee's nose.

Employee was removing roof bolts from scoop bucket and received a cut from the metal band that binds the roof bolts for transportation. Employee received 4 sutures and has not lost any work days.

The cutter head on the miner failed. So the operator backed it out of the cut. EE began removing the rub-rail to gain access in the control box. He tried to sit the rail over the miner cable. Something pull in his shoulder. He was diagnosed with a portial tear of his rotator cuff. Note: he was not diagnosed until 3/24/08.
