Stockburg No 2Coal

A roof Fall was discovered along the Buckshot #1 belt-line in the intake. The fall did not affect/impede passage or ventilation. All entries to the fall have been timbered/dangered off to prevent travel.

At 4:00 am a roof fall was discovered in the returns along the #2 Buckshot belt-line. Spad # 02252. All approaches to the fall have been cribbed or timbered off to prevent travel.

Pinched finger between motors

Miner was hanging miner cable and strained lower back. He did not start missing work until 5/29/08

When the day shift production crew arrived at the section, the boss discovered a small roof fall at approx 7:50am. The fall measured 10' long, 6' tall and 6' wide.

The cutter head on the miner failed. So the operator backed it out of the cut. EE began removing the rub-rail to gain access in the control box. He tried to sit the rail over the miner cable. Something pull in his shoulder. He was diagnosed with a portial tear of his rotator cuff. Note: he was not diagnosed until 3/24/08.

Miner was pushing a bolt into the mine roof with the bolting machine. The wrench and bolt were mis-aligned, so the miner bumped the controls to rotate the wrench. His glove was pinched between the wrench and bolt, jerking his hand into the spin. His ring & pinky were dislocated.

Employee was taking continuous miner power cable down when he felt a pain in his neck and back.

Employee was pulling up on a 15 inch adjustable type Cressent wrench when he felt something pull in his left shoulder. He started missing work on 10/17/07.

Employee was using a pick hammer and masonry hammer to remove a mechanical splice. He struck the pick hammer with the masonry hammer and a piece of the pick hammer splintered off.

Employee was riding on top of battery compartment on scoop & was thrown against mine roof.

Employee was using hammer & chisel to cut head off of bolt & a piece of the chisel flew off embedding in his left index finger.

Employee jumped off mantrip before it collided with another mantrip.

EE had his left hand on the drill pot as it was being raised. His left little finger got caught between the leveling arm and feed jack cylinder.

Employee struck the rib while operating a scoop. He claims to have injured his back.

EE struck his thumb while putting a bit insert into a miner.

EE fell @ belt head. He didn't trip- just felt pain in his back.

Employee claims to have injured his back while picking up flights for a conveyor chain. He did not start missing until 2/8/2007.

EE suffered a muscle strain while moving a feeder cable out of the way. He did not start missing work until 1/17/2007

EE jammed his head into a scoop canopy while hauling block. He had run over a track rail when this occurred.

Miner drum slid out of scoop bucket and hit employee on his foot(R).

A piece of metal flew off of a hammer & struck employee in his right arm.

While shoveling a belt, employee was struck by a belt scraper. He then fell against the rib & his rescuer.

Employee inadvertently cut his right knee while cutting belt. THIS IS NOT A LOST TIME ACCIDENT, BUT REQUIRED SUTURES.

Employee was letting down wire for powerline and strained stomach muscle.

Employee cut his hand (L) while cutting a piece of belt that was to be used for spill rubber.

Employee fractured his little finger (L) when it was caught between the level wind & the mounting bracket.

Employee was pulling the plug out of the #2 scoop. The plug slipped & he fell back & struck his hand on the lightguard.

EMPLOYEE WAS WORKING OUTBY AND TOOK OFF HIS SAFETY GLASSES TO WIPE SWEAT FROM HIS FACE AND WAS UNAWARE HE HAD B-BOND ON HIS SLEEVE WHICH GOT IN HIS RIGHT EYE.

Employee was struck by a coal chute that he was disassembling.

Employee was looking for an oil leak on the continuous miner. He pulled the hose & oil shot into his right index finger causing it to swell.

Employee struck his head on the shuttle car canopy when he hit a hole in the roadway.

Employee cut his finger while using a hack saw to make a splice in a waterline. This cut required sutures.

While removing a wrench from a drill steel, the wrench fell & mashed his finger against the drill pot.

An unintentional roof fall occurred 1 break inby spad # 963 on the Old 1st. Right Section. Measurements: 20'(L) x 20'(W) x6'(H).

Unintentional roof fall on 1st. Right Section, 1 break to the right of spad # 1573. Measured: 20' (W) x 20'(L) x 6'(H).

While roof bolting, a rock fell from the mine roof & came off the bolter's canopy & struck employee in his lower back.

Unintentional roof fall in # 7 Entry @ spad # 1646. The fall measured 20' x 20' x 7' to 8' thick.

Employee was struck & knocked down while he was in the process of taking down a high voltage cable. When he cut the strap that was holding the cable- the cable fell on him.

Employee was struck by a piece of rock that fell from the mine roof while he was changing inserts near the back of the roof bolting machine.

Employee was working on a drive shaft on a mantrip. The mantrip moved & his hand was injured.

Unintentional roof fall 20'(W) x 25'(L) x 8'(H). 20' inby spad # 1597.

Employee alleges to have injured his back while dodging a piece of draw rock that had fallen from the mine roof.

Employee was struck by a piece of mine roof that fell from between the roof bolts & rib & struck employee on his shoulders. He was roof bolting & had bent over to pick up a tube of resin that he'd dropped at the time of the accident.

Employee was struck by a rock that fell from in between roof bolts.

While roof bolting, employee set the foot jack which broke thru a piece of rock. The rock then struck him on the top of his foot (L).

CUTTING BANDS ON KENNEDY STOPPING & RAZOR KNIFE SLIPPED, CUTTING HIS FOREARM.

EE was struck by a piece of rock that fell from between the roof bolts. He was walking to the front of the miner to help set bits at the time this accident occurred.

The mantrip motor that employee was working on slipped off its blocking & mousetrapped his little finger between the frame & blocking.

EE was struck by an Omega Block while building a stopping. He had installed a door and walked through it & a block fell on him (pulled muscle in neck). EE started missing work on 4/15/05.

EE WAS AWARDED 0.55% PPD BENEFITS FOR NOISE-INDUCED HEARING LOSS.

EE injured his back while using a slate bar to clean rock off of a continuous miner. HE DID NOT BEGIN MISSING WORK UNTIL 3/17/05

ground gave way - fell over 6 ft ledge - suffered a compound fracture at his left elbow

While bolting, employee's glove became entangled by drill steel & twisted his hand.

Operator was pulling a dozer out of a stuck position - got dozer out - hit the tram leavers - was back further that he thought - excavator flipped backwards & rolled - landed on the cab - cab glass was broken - removed broken glass & got ee out

(2)EMPLOYEE'S WERE WORKING ON CHANGING ROLLERS ON CONVEYOR. (1)EMPLOYEE LEFT TO GET TOOLS & THE OTHER EE FELL OFF CONVEYOR & INJURED HIS LEFT THUMB. BOTH EE'S WERE WRITTEN UP FOR FAILING TO WEAR FALL PROTECTION & A SAFETY MEETING WAS HELD.

EE was attempting to remove a broken belt roller. The belt roller came into contact with the belt & then struck the employee in the face.

Employee slipped on loose material while walking beside a power center. He injured his finger when he fell into power center.

EMPLOYEE WAS LOWERING BELT LINE WITH HIGH LIFT JACK. THE JACK SLIPPED OFF OF THE STRUCTURE, DROPPING THE STRUCTURE ON HIS RIGHT LITTLE FINGER.
