At 4:00 am a roof fall was discovered in the returns along the #2 Buckshot belt-line. Spad # 02252. All approaches to the fall have been cribbed or timbered off to prevent travel.
Miner was hanging miner cable and strained lower back. He did not start missing work until 5/29/08
When the day shift production crew arrived at the section, the boss discovered a small roof fall at approx 7:50am. The fall measured 10' long, 6' tall and 6' wide.
The cutter head on the miner failed. So the operator backed it out of the cut. EE began removing the rub-rail to gain access in the control box. He tried to sit the rail over the miner cable. Something pull in his shoulder. He was diagnosed with a portial tear of his rotator cuff. Note: he was not diagnosed until 3/24/08.
Miner was pushing a bolt into the mine roof with the bolting machine. The wrench and bolt were mis-aligned, so the miner bumped the controls to rotate the wrench. His glove was pinched between the wrench and bolt, jerking his hand into the spin. His ring & pinky were dislocated.

Employee was taking continuous miner power cable down when he felt a pain in his neck and back.
Employee was pulling up on a 15 inch adjustable type Cressent wrench when he felt something pull in his left shoulder. He started missing work on 10/17/07.
Employee was using a pick hammer and masonry hammer to remove a mechanical splice. He struck the pick hammer with the masonry hammer and a piece of the pick hammer splintered off.
Employee was riding on top of battery compartment on scoop & was thrown against mine roof.
Employee was using hammer & chisel to cut head off of bolt & a piece of the chisel flew off embedding in his left index finger.
Employee jumped off mantrip before it collided with another mantrip.
EE had his left hand on the drill pot as it was being raised. His left little finger got caught between the leveling arm and feed jack cylinder.

EE jammed his head into a scoop canopy while hauling block. He had run over a track rail when this occurred.

EE was struck by a piece of rock that fell from between the roof bolts. He was walking to the front of the miner to help set bits at the time this accident occurred.
The mantrip motor that employee was working on slipped off its blocking & mousetrapped his little finger between the frame & blocking.
EE was struck by an Omega Block while building a stopping. He had installed a door and walked through it & a block fell on him (pulled muscle in neck). EE started missing work on 4/15/05.

EE was attempting to remove a broken belt roller. The belt roller came into contact with the belt & then struck the employee in the face.
Employee slipped on loose material while walking beside a power center. He injured his finger when he fell into power center.
EMPLOYEE WAS LOWERING BELT LINE WITH HIGH LIFT JACK. THE JACK SLIPPED OFF OF THE STRUCTURE, DROPPING THE STRUCTURE ON HIS RIGHT LITTLE FINGER.
