While mixing copper sulfate, some of the solution splashed onto employee's face and under his safety glasses into his left eye. He was released full duty. In reviewing the file it was recogonized he was given presciption medication for comfort which makes this reportable.

The EE was carrying two roof bolt plated in his hand and received a cut from a piece of metal protruding from one of the plates.

The jumbo operator leaned a 5' x 10' wire up against the rib to paint it for bolting. He had the wire too vertical and when he walked away to get another wire, the wire fell forward and struck him on the arm causing laceration/puncture to his left arm elbow.

Miner was walking down 5 north when he felt a shootingpain in his lower back. The miner had picked up a prill pot earlier in the day and thought his back was reacting to that. He was taken to the doctor adn was placed on restricted duty.

While mounting a 950X20 tire, picked up tire to rotate and install on equipment, lifted to mount Right Frt tire, felt a pop in back, and could not bend over afterward.

HELPER WAS PUTTING SOME CORE AWAY, HAD TURNED AROUND & WHILE HIS BACK WAS TURNED SOME RODS THAT WERE STACKED UP FELL, STRIKING HIM IN HIS RIGHT RIB CAGE.

While working on drill around 9:30 pm I felt a pain in my back (upper left side). I continued to work but after awhile I could not lift even an empty tube. The pain had started to go down my left arm & halfway up my neck. I informed driller & we went to hospital.

THE EMPLOYEE THOUGHT THERE WAS AN EYELASH IN HIS LEFT EYE. HE RUBBED IT. THE IRRITATION DID NOT GO AWAY. HE WENT TO THE DOCTOR TO HAVE IT LOOKED AT AND A PIECE OF METAL WAS REMOVED. HE WAS GIVEN A PRESCRIPTION FOR ANTI-BIOTICS AND RELEASED TO FULL DUTY.

Employee was rehanging some ventilation in 5 haulage. His partner lifted the vent bag and some dust fell off the bag and went into his eye. The employee was wearing prescription safety glasses while working on the vent bag.

Employee was lifting a 40 lbs bag of anfo to dump into a prell pot. He was on his 4th bag when he felt a constant pressure and discomfort in his abdomen.

Operator was removing brick from a furnace. On raising his faceshield particles fell across his forehead, bounced off glasses into right eye. Eye was flushed & operator transported to town. Seen by doctor. Eyedrops prescribed for scratch on the sclera. Temporary irritation. Miner returned next shift.

EE injured his back while attempting to move a fender that had been knocked off a truck. He continued to have pain and was taken to medical clinic on 4/4/05. He was diagnosed with back spasms and was given prescription muscle relaxers and was teaken off work for 2 days.

Miner was unloading wire mat off flatbed. A wire caught on the bundle and when he released it, the wire slid off the truck and landed on his foot. He received a puncture wound. He was given antibiotics by the doctor the next day and was given a full duty work release for his next shift (3/25/05).

While replacing a seat on a 6 yard loader, the base caught on some hoses forcing the seat down with employee's thumb under it. The mechanic was seen at the clinic and was given restricted duty dut to a sprained thumb.

While fabricating a roof for structure, the frame for the roof slipped. It caught the index finger pulling off the nail. First aid administered and he was taken to town. No fracture but antibiotics prescribed.

WHILE PLACING/DRILLING GROUND SUPPORT BOLTS AND WIRE THE INJURED PULLED THE LEG OF HIS JACKLEG DRILL BACKWARD AND CAUGHT HIS LEFT ARM ON A PIECE OF THE GROUND SUPPORT WIRE. THE INJURED RECEIVED A LACERATION THAT REQUIRED TEN SUTURES TO CLOSE THE WOUND.

Ee was attempting to remove brake cover from 40 ton truck. He removed all bolts but 2. He supported cover on his knee then removed last 2 bolts. He put his hand on inner side of ring & moved his knee. When he moved his knee the ring fell thus smashing his left hand between ring & spindle. Resulting in broken middle finger & ring finger. Middle finger also required 8 stitches.

SUBJECT WAS CLOSING THE DOOR OF A UTILITY P/U, MISJUDGED THE DISTANCE AND DID NOT REMOVE HIS HAND IN TIME PINCHING THE DISTAL PHALANGE OF THE INDEX FINGER OF THE LEFT HAND BETWEEN THE DOOR AND THE TRUCK CAB. NO PERMANENT DAMAGE OR IMPAIRMENT EXPECTED.

EE FELT A SHARP PAIN IN HIS NECK & RIGHT SHOULDER WHEN LIFTING ARMS CHEST HIGH. THE PAIN WAS OFF & ON THE REST OF THE SHIFT. NOTE: REVISITED DR ON 5/21/04 & RECEIVED WORK RESTRICTIONS.

THE MINER WAS INSTALLING WIRE. HE HAD PROPPED THE WIRE TO HOLD IT WHILE HE PREPARED TO DRIVE ANOTHER BOLT, HE STARTED THE BOLT IN THE HOLE BY HAND, HE BENT DOWN TO GET THE DRIVER. THE PROP SLI PPED. AS THE WIRE DROPPED IT POPPED THE BOLT FROM THE HOLE. THE BOLT FELL STRIKING THE MINER CAUSING THE CUT TO THE MINERS EYEBROW.
