Employee was stripping worn metal from a 988B loader bucket. One of the pieces of metal came loose and fell, striking the employee in both feet. The incident resulted in a fracture to his right foot. The employee did not seek medical attention until 01/10/08. Surgery to place a pin is scheduled for 1/17/08.

Showing all 6EE was using a degreaser to cleanout an oil tank. As he sprayed the solvent into the tank, he inadvertently inhaled too much of the fumes. He became nauseated and requested medical attention. The physician prescribed medication for nausea.
Trying to replace the pin in the linkage arm on 972 wheeled loader, when the bar they had in the hole in the arm, slipped out, letting the arm fall. The linkage arm hit EE in the right ear and left shoulder.
EE was pulling on v belts to turn crusher to align bolts for drive tires.
Employee was assisting in replacement of impeller for classifier pump. as they were putting pump back in place, the pump swung out knocking the board he was standing on loose. he grabbed the motor to stop his fall. the motor fell into classifier and when it came to the end of the chain it stopped abruptly, causing the motor to strike the ee s chest
Lifting 4'x 5' 2" screen to replace broken scrapes when the screen slipped out at his hand hitting him on the right foot.
