Fall to the walkway or working surface

Employee suffered from suspected Heat Exhaustion. At some point after exiting the enclosed cab of the excavator, employee fell. Employee was found conscious but disoriented on the ground. After falling, EE struck EE's head on an unknown object(s). Employee doesn't know how the fall occurred.

Bodily reaction, (Not Elsewhere Classified)

Employee was sitting in control building at the jaw crusher. He states he turned his head to watch material coming from the feeder bin and he felt his neck ""pop"". The incident resulted in a neck and trapezius strain.

Struck by flying object

Crew was working on feed chute to cone crusher. One of the crew was positioned above the injured employee using a grinder. Metal debris from the grinding struck the employee in right eye. Doctor brushed out debris and prescribed anitbiotic ointment.

Struck by falling object

Employee was stripping worn metal from a 988B loader bucket. One of the pieces of metal came loose and fell, striking the employee in both feet. The incident resulted in a fracture to his right foot. The employee did not seek medical attention until 01/10/08. Surgery to place a pin is scheduled for 1/17/08.

Absorption of radiations, caustics, toxic and noxious substances

EE was using a degreaser to cleanout an oil tank. As he sprayed the solvent into the tank, he inadvertently inhaled too much of the fumes. He became nauseated and requested medical attention. The physician prescribed medication for nausea.

Lifting 4'x 5' 2"" screen to replace broken scrapes when the screen slipped out at his hand hitting him on the right foot.
