On 8/8/14 the employee was unable to return to work at full capacity due to an injury that occurred on 6/1/14. On 6/14/14 the employee attempted to insert a roof bolt into the drill hole and shove the bolt through the resin cartridge by hand. The action strained his right shoulder.
The employee attempted to pick up a bundle of roof bolt plates that had been spilled to the ground. A bundle of plates on top of the pile moved and pinched the employees left index finger. The employee was working his last shift before an extended leave of absence but was released to full duty.

The finger was injured on 5/28/13 when the employee removed the U-bolts from the axle of a welder trailer and his hand was pinched between the trailer frame and the axle hub. The ligament damage to the finger has not healed properly
On 9/16/13 the employee had surgery to reduce a hernia that may have been started on 3/14/13 when the employee was lifting bags of Tek-Seal onto the pump for mixing.
