Roof strap was broke hanging down, principal was bending strap back and forth trying to break it off when he hit his right arm on the other end of the strap causing a laceration requiring 8 sutures.
Working on a head drive and was attempting to install drive sprocket. He had it blocked up on shaft and was sliding sprocket inline and it slipped off shaft, causing his hand to be between sprocket and blocking material. This resulted in 3 stitches to his hand from a laceration.
Shoveling out from under #3 belt take up with long handle shovel when pulling shovel back out with gob on it flipped shovel over felt pain in shoulder. Started missing work on 2-17-2012.
Employee was bolting an open break between #6 and #7 entry. Employee went to bend a bolt and felt a sharp pain in his lower back and tail bone. Employee stated that the pain went down to his left leg.
Loading straps and got cut on piece of metal that was on the straps.
She got caught her body stuck while cleaning out material at a head drive. In trying to get her body unstuck she claims that she bruised her ribs and shoulder.
He moved shuttle car anchor up to the corner of the block, he twisted around to pick up the anchor and felt something pull in his back.
He and another repairman was lifting a metal cover on the continuous miner, when he slipped and the metal cover pinned his arm against the continuous miner frame. This resulted in a fracture of the small bone in his right foreman.
EE was getting a cleanup car, when a rock came out of the conveyor chain of a continuous miner and struck him in the right jaw and behind the right ear. He was x-rayed and determined to have no fractures, but he did have a laceration that required 4 sutures.
Cleaning back board off at #2 tailpiece with a roof bolt, when the bolt came in contact with #2 belt, pushing his hand into the mine roof. Cutting left hand and requiring stitches.
While trying to free up the brake on a mine buggy, he had to jerk his hand back suddenly and came in contact with a nail which caused a laceration to his left middle finger.
He was trying to get the top piece of drill steel out of the hole that he had just drilled. He had his left hand on the drill pot, while working on getting the steel out of the hole, when he pulled the bottom piece out of the hole the top piece of steel came out and hit the left index finger.
Taking a metal bar out of the center of a 1000 roll of belt, as the bar was being let down the bar fell on his middle finger on his left hand. This resulted in a fracture of the end of that finger.
Had loaded a continuous miner on the lowboy, while taking the block off the stand, EE tripped the stand release and the stand came up and caught another ee left hand between frame and stand.
He was working on constructing an overcast and was handling a metal rail to put in the overcast. The rail fell making contact with his hand against the bucket of the scoop. This resulted in a laceration to the palm area of his hand and fractured bones in the hand.
Employee was dropping high-line from the top. As he dropped the high-line he got his ring finger on his left hand caught in a hook of high-line hanger causing a laceration in his finger. Received 5 stitches.
Roof Bolter Operator bending roof bolt and felt back "pop".
Was putting up a pogo stick to hold down a ventilation curtian when he felt pain in his left hip and leg.
He was trying to pull cable off of roof bolter, when he felt pain in his lower back. He did not start missing work until 9/2/09 because of incident.
Placing beams against mine roof with jacks, when one of the jacks kicked out, throwing the beam sideways and striking EE. He had to have 6 stitches for a laceration above his left eye.
While loading roof bolter with supplies a metal strap on a bundle of rope bolts was broke and sticking out as he turned to put supplies on bolter left arm caught the broke strap cutting his arm.
Was pulling miner cable out of roadway for miner to move across section, when he experienced pain in his lower back and hip. He went to E.R. at McDowell Hospital, was x-rayed and was told to see family doctor. Started missing work on 5/18/09.
While throwing Miner Cable out of roadway for roof bolter to cross over, felt something give in lower back.
EE was straightening a 5' bolt after it was inserted in the top. The bolt turned to the side and he then felt something pull in the lower part of his back.
Employee was getting into a 3/4 ton pick up truck and while he turned to cough, He felt severe pain in his chest and it made a cracking noise. He was taken to medical clinic to be checked and found that he had two fractured ribs.
EE was loading a drive chain for feeder onto buggy and felt pain in lower back.
He got a roof bolt hung up in the hole and was trying to bend it to break it off. The roof bolt flew over against his right arm.
He was putting a roof bolt up into the hole and was trying to unbend it and the roof bolt twisted. He felt a sharp pain in his shoulder.
EE was lifting his tool box onto a "buggy". When he twisted to load the box he experienced pain in his groin,left work due to this pain and since then he has complained that he had pain in his lower back.
Surface employee was loading buckets of mine sealant into the supply car, when he dropped a bucket. The lid came off of the bucket, allowing some of the sealant to splash in his eye. This resulted in a scratch to his cornea.
Employee stated that when he picked up a bucket of oil and then later unloaded his tools his back became aching and stiff. He had a previous back injury and this incident has been under investigation. It has now been determined to be a new injury and is now reportable.
Employee had stepped up on to a scoop in the shop to hook the battery charger up when he got his right foot caught between the steering jack and the frame fracturing a bone in his foot. He is working restrcited duty.
Employee was picking up a conveyor belt roller when he pulled a muscle in his back.
he was working on seals and dropped a concrete block on his finger(left hand small finger) he did not stop working and he went to the doctor sunday. it is not a loss time accident.
Employee was removing a metal rub rail guard from the continuous miner when he accidentally dropped it on his right foot. This resulted in a bruise and sprain to his foot.
Employee was assisting his fellow roof bolter opertaor in hanging the roof bolter cable from the mine roof using insulated tie wire. When he turned his head to move, the excess end of the wire struck his right ear and actually went down into his ear. This resulted in a possible puncture to his ear drum.
Employee, was removing a metal cover from a front end loader when he received a cut on the palm of his right hand that required 6 stitches to close.
Employee was lifting pieces of belt structure when he injured his back. He continued to work until 8-11-05 at which time he felt he needed to see a doctor due to the fact that his back was continuing to hurt. He was diagnosed with a pulled muscle and was told to take a couple of days off. He missed two days of work.
EE was removing a piece of fallen draw slate from off of the belt conveyor when he got his small finger on his right hand pinched, resulting in a laceration that required stitches to close.
Employee was assisting fellow employees build cribs when he got struck on his left hand with a crib block. This resulted in a laceration on his index finger that required 6 stitches to close.
Employee bent over to pick up a wooden crib block that weighed approximately 20 pounds when he felt a sharp pain in his lower back. Employee continued to work until he decided to seek medical attention a couple of days later on 02/21/05 and is now missing time due to the injury.
