Employee was using a claw Hammer to drive a nail missed the nail and hit his index finger on his left hand.

Employee was getting into a 3/4 ton pick up truck and while he turned to cough, He felt severe pain in his chest and it made a cracking noise. He was taken to medical clinic to be checked and found that he had two fractured ribs.

Employee was dismounting the D9R #2 Dozer with an oil sample bottle in his hand, when his foot slipped, causing him to strike the blade push bar with his left rib cage.
