Employee was dismounting the D9R #2 Dozer with an oil sample bottle in his hand, when his foot slipped, causing him to strike the blade push bar with his left rib cage.

Employee was closing doors on the rail cars when he slipped twisting his ankle resulting in a fracture in his ankle.

Employee was bending over to tighten a clamp on a pipe bonder when he felt pain in his back. Employee did not miss any work until 09-17-07. Accident has been under investigation and has been determined to be reportable.

Employee slid off the side of a belt tube and twisted his ankle. He was attempting to line up a seam in the tube so a connection could be made with the band clamp.

Employee was tightening bolts and strained left elbow.
