Employee was moving a fallen timber out of the walk way on 3A belt. When he picked the timber up he felt something pop in his lower back.
EE picked up a tool bag and felt pain in his left shoulder. He went to the doctor on the 15th and they gave him a full release. He went back to the doctor on 6/19/15 and they gave him a light duty slip but we didn't see it until 7/7/15 and he started missing work on 7/8/15.
Employee and three other was hanging miner cable in # 7 entry and as Employee lift the cable he felt pain in the lower back.
EE had been working on a ventilation curtain in #1 heading he completed task, he removed his safety glasses to wipe sweat of his eye and rubbed dirt in the left eye causing irritation. EE stated that he had mud on his hands from the ventilation curtain. EE started to miss work on3/31 thru 3/30 and returned to work on 3/31/15. EE worked tow day before going to the Dr.
Employee was putting up a 12 foot cable bolt and as he went to put the bolt in the hole he felt pain in his left shoulder. The injury result in restricted work for 17 days, employee started missing work on 11/20/2014. (The mine was down for the week of Thanksgiving for vacation.)
Employee was removing 3" waterline that was hooked to the hydroseeding truck. Pump that was hooked to truck was on automatic. When the employee unhooked the line the pumped kicked on and the hose struck the employee in the neck causing a laceration requiring stitches.
Employee was trying to stabilize the ATRS beam mount to flip it over. When flipping it over the mesh latch assembly came down and cut from the first knuckle off on his index finger on left hand.
Moving miner cable out of the roadway tripped over the miner cable and jammed left little finger. (EE worked from 13th to the 16th before going to the Dr. to have his finger checked. He went on lite days on Monday the 19th and return to full duty on the 27th)
Employee was in the process of pushing a flat car over to put back on the rail with a hydraulic jack. The jack kicked out and the bar hit him on the forehead. He still had the bar in his hand. He received 3 stitches.
He was changing out a spring on a vibrator and he got his finger in between the spring and pedestal cutting it and he received stitches.
The employee thought he put the scoop bucket on the ground, but it was setting on a big piece of coal. While he was loading the block in the scoop bucket the piece of coal burst out and scoop bucket hit the side of his right foot.
Employee was grabbing cable spinning hoist and his little finger -(RH) got stuck in hoist.
EE was closing the elevator door in the prep plant when he pulled the strap to close the door. The door caught his finger in the door channel causing the injury
Loading boards on roof bolter and hit his left arm again a stack of pizza pan and cut his arm above the elbow.
EE was hooking up a sling duster to the scoop bucket as he went to tighten up the chain the duster fell off on his right foot causing a simple fracture to it.
EE was changing out a top roller on a 60" belt and he was standing closer than he was aware of and hit his little finger between the roller and the belt rail.
Employee began missing work on February 27, 2013 after he started to have trouble out of his right shoulder that he injured on Dec 18, 2012 while hanging a miner cable. (Employee had a pre-existing non mining related injury and had surgery to the right shoulder).
EE was putting a 6' roof bolt in the drilled hole when he went to straightening the bolt out it slipped and came back and hit him in the hand cutting his finger. EE received 6 stitches. He was wearing knucklehead X10 gloves.
EE was taking down a buggie cable when he unhooked the hanger the hook came around and hit him in the nose and cut it he received 7 stitches.
EE was getting a piece of draw rock off the miner and it slid across his hand cutting it. The cut took 7 stitches.
This is an old injury that happened on April 4, 2012. EE had been going back to the doctor several times since the injury. EE started missing work on October 26, 2015 for surgery on his left shoulder.
While loading shuttle car a rock slid off the car hitting him on the left ankle.
Employee holding a piece of curtain up for shuttle car to pass thru when a lump of coal came out from under tire hitting his Rt. knee. He went to ER on Dec 13,and was given off 1 week. He followed up with specialist on 12-19-11 and released to return to full duty. He started his scheduled vacation on Dec.16th and not scheduled to return until(12-27-11)from vacation
EE was cleaning out the scrubber on the miner and the scrubber door fell and hit him on the left index finger. EE received 5 stitches.
Employee was loading supplies on the tray of the bolter. As he was reaching across some roof straps to get the glue, the palm of his right hand hit the end of the strap cutting it. EE received 4 stitches.
While lowering middle belly pan on dozer, sling slipped and pan fell against right arm causing laceration.
Employee was dropping a cross under guard loose from a belt line. The chain that was holding the guard in place was wrapped around a 4 inch water line. He dropped the guard and the chain unrolled very fast hitting him in the face. Employee received 3 sutures to the nose.

While he was building a stopping he was getting a block off the pallet and one fell off hitting his right thumb. (He returned to work on his next shift 7-20-09. This was a medical report only.)
While the employee was putting belt structure in on a belt move, he felt something pull in his right groin area.** Employee started missing work on 6/3/09.
The employee was pulling up his clothes basket and the one beside it fell hitting him in the front part of the head. The cut required 7 staples.
EE was trying to remove a hose from a rock duster an step on the hose with his right foot and turn it over, braking above in it.
The employee was cleaning the scubber out on 95 miner. He had the scubber hood up and the side door open cleaning inside the scrubber. The scrubber hood fell on his left hand cutting it. He received 14 stitches.
Employee was changing a cutter torque shaft on #51 miner and was putting the shield back on. A piece of rock fell out of the top hitting him on the left forearm.
Swinging plates removed from washer area to outside, turned to swing plate and twsited right knee.
Employee said a miner cable fell off the hook and he tried to catch it and it hyper-extended his right thumb.
EE was shoveling belt.
Employee was loading a drill feed jack on a pallet with a forklift. He go off forklift to shove jack off forks to a pallet and jack slid off forks striking employee on left leg and ankle. He also complained of lower back pain. He started missing work on 2/20/06.
While working on #51 sump on the first floor, the employee had to shut down the #64 sump and forgot to shut down the sulfuric acid pump. Employee went to the second floor to drop a water hose to the first floor and the fumes from the acid took his breath.
