Occupational hearing loss filed 1/7/2016. I received the e-mail on 4/19/2016. The employee worked up to when the plant closed down 11/11/15.

Employee was diagnoised with pneumoconiosis by a board certified doctor in internal medicine on 11-10-2015 we recieverd the Email on 06-10-2016.
Employee was working on 53 screen changing out head board rubber. As he went to step up on the screen his foot slipped and he fell to the floor hitting his head and right elbow on a piece of pipe. Employee received 4 stitches to the head and 3 on the right elbow.

Employee was removing 3"" waterline that was hooked to the hydroseeding truck. Pump that was hooked to truck was on automatic. When the employee unhooked the line the pumped kicked on and the hose struck the employee in the neck causing a laceration requiring stitches.
He was changing out a spring on a vibrator and he got his finger in between the spring and pedestal cutting it and he received stitches.

Employee was grabbing cable spinning hoist and his little finger -(RH) got stuck in hoist.
EE was closing the elevator door in the prep plant when he pulled the strap to close the door. The door caught his finger in the door channel causing the injury

Employee was getting off elevator and caught his right hand between the doors. The employee continued working until April 12, 2012 when he went to the doctor to get it checked.
Employee was conducting Maintenance on a D6R Catterpillar and during the removal of the belly pan the come a long chain broke and the pan hit his right forearm. Also, the pry bar being used struck his right ring finger causing a cut.

EE was stepping down from a endloader onto the ground and his right foot turned sideway and he sprain his ankle. EE when to the doctors about two weeks later and had a simple fracture to the foot. (This was over look and not turn in until now).

The EE was going up a set of steps and the tread broke cause him to fell backward and landing on the floor.
EE stated he was changing screens in shaker, got finished & put left leg over front of shaker. He got boot caught on bolt causing other foot to slip, resulting in him hanging by boot on left foot fot 2-3 seconds until he got boot loosened.
Washing down sump with a water hose while standing on a platform. Slipped and fell approx 18"" striking back on a crib block and handrail.

EE was trying to unstop a 4"" valve. While hitting it with a handle, he hit his right index finger, cutting it. They put 7 stitches in it.

Swinging plates removed from washer area to outside, turned to swing plate and twsited right knee.
EE was on top of beam, pipe fitting, stepped off of beam onto round tube with wire mesh on tube. Wire mesh caught on boot, when he tried to step off, it caused him to fall and wire mesh stuck into left leg.
Employee while reaching screen up to change out on #3 screen on 115 pre-wet floor, slipped and fell backward injuring his neck. Although this accident happened on 4/26, employee never started missing work until 5/19/06.

Working on wash box, stepped off ladder onto sledge hammer handle & injured his leg.
Three employees were moving a pallet jack from the first floor to the second floor in the prep plant elevator. As our employee and one of the contractors was closing the first set of verticle doors on the elevator, the bottom door caught the edge of the pallet jack lifting it up and sliding off striking the employee's left foot.
While working on #51 sump on the first floor, the employee had to shut down the #64 sump and forgot to shut down the sulfuric acid pump. Employee went to the second floor to drop a water hose to the first floor and the fumes from the acid took his breath.
The employee was working in an area with low overhead room. The steel struture was low. The employee repeatedly raised striking his head on the low overhead steel throughout the shift. The employee reported the injury and sought medical attention on Friday, Fenruary 25, 2005.

Working on 1st floor of prep plant running a grinder and something went into left eye, had safety glasses on but still blew a foreign object into eye. he didn't think it was serious until the eye didn't get any better.
EE WAS REPAIRING RAW COAL SCREEN. HE STEPPED ON A PIECE OF COAL BESIDE SCREEN AND TWISTED ANKLE.
GRINDING ON A PIPE, SOMETHINE FLEW UP AND WENT UNDER SAFETY GLASSES INTO RIGHT EYE.
Putting motor sheave on pump and pinched fingers between sheave and belt resulting in 2 cut fingers.
INSTALLING 400 LB. PIECE OF METAL WHEN IT CAUGHT, CAME LOOSE WHILE MY HAND WAS BEHIND IT CRUSHING MY FINGER.

THE EE SLIPPED AND FELL WHILE WORKING AROUND THE UNIT 736 DRYER. HE CONTINUED WORK TILL THE END OF HIS SHIFT. THE EE TOOK A WEEK OF SCHEDULED VACATION AND BEGAN MISSING TIME ON 5-19-03 DUE TO THIS ACCIDENT.

WHILE LIFTING A BAG OF COAL INJURED PERSON FELT A SHARP PAIN IN HIS BACK.
CUTTING METAL WITH TORCH ABOUT 20 MINUTES LATER HE RUBBED HIS EYE AND STILL HAD HIS GLOVES ON. EYE STARTED BURNING AND IT SWELLED. APPEARS WHEN HE RUBBED HIS EYES SOMETHING OFF OF THE GLOVE GO T IN HIS EYE.
