Employee got his middle finger caught between the keeper latch & pin while removing the pin. Cause of injury was lack of attention. Miner proficiency may have been a factor. PPE, company rules or mining equipment was not a factor.

Employee was standing in water being treated by chemicals. Cause of this injury was attitude/holes in rubber boots. Miner proficiency, PPE, and company rules was a factor. Mining equipment/systems was not a factor.

Metal trailer tongue slipped off hitch, fingers were still on the torque, pinched fingers between tongue and crib block. Cause was not moving away after placing tongue on hitch. Compliance with company rules a factor, miner proficiency may be a factor, protective equipment not a factor, mining practices could be a factor.

Pin was being removing from a pusher block on the highwall miner launch using a pry bar. He squatted down under the launch, pulled on the pry bar and felt discomfort in his groin. Cause was poor judgement in the means of removal of the pin. Rule compliance not a factor, mine equipment not a factor, proper clothing not a factor and miner proficiency and attitude may be a factor.

Employee was getting ready to install a roof bolt. He had to place headers in a void in the top above the strap and as he stepped down, he struck his forearm against the bolt plate, cutting his wrist. The laceration required 10 sutures to close the wound.

Employee was cleaning mud off a miner bit. A metal burr was protruding from bit and his left thumb caught on the burr and lacerated his thumb. Employee had to have four stitches and returned to work.

EE was attempting to put antifreeze in the "Gen Set". When he removed the cap from the tank, air pressure blew cap upward, lost my balance and fell striking the tank. Cause of this incident was the lack of awareness and not releasing pressure on tank. Compliance with rules & miner proficiency a factor, PPE, mining/systems not a factor. Filed due to a part 50 audit.

Employee was pulling on female cathead bracket on a highwall miner conveyor car. The hinge pin was rusted, so he pulled hard on the bracket lever to free the bracket. He felt pain when he jerked on the lever. Shoulder strain.

Employee was attempting to use a conveyor car connecting pin to push mud out of the conveyor car coupler. As he was tapping the pin against the mud in the coupler hole, his finger struck against the pin keeper, lacerating the finger. Three stitches required to close wound.

Employee was pulling a connecting pin on a conveyor car on the Addcar highwall miner sysytem. The pin was bound at the connecting point so the employee jerked on it to pull it free. When he jerked on it, he felt pain radiate across his lower back. Physician evaluation was a strained lower back.

Employee was picking up a large clevis. As he was reaching to support the clevis with his right hand, a metal sliver (burr) went through his glove and punctured his hand. The imbedded object had to be removed by a doctor.

EE was removing the cap from the cathead of the 4160 power cable at the generator set. He spun the cap loose but as he pulled on it, it abruptly released from the threads and struck him in the mouth. His front tooth was chipped by the cap. It will need repaired by a dentist.

Employee was helping another employee lift a rock from the conveyor car on the High Wall Miner. The other employee lost his grip on the rock and the rock dropped onto the frame of the car. The employee who was helping remove the rock, pinched his finger between the rock and frame of the car. The first digit of his ring finger was fractured.

Employee was openning the access door to the crusher to check for blockage. After removing the bolts on the upper section of the door, he started to swing the door open. The hinge pins abruptly broke and the door dropped to the floor, striking the employees foot. Employee's foot was fractured on the metatarsal area of the foot. Employee was wearing metatarsal boots.

Employee was placing power cable for a highwall miner conveyor car into the channel on the car. As he was aligning it, the cathead on the power cable pinched his right index finger between the cathead and the conveyor car frame. His finger was fractured.

Employee was attempting to dislodge a lump of coal from between the rear set of dual tires from a contractor's coal truck. As he hit the coal with a hammer, one of the tires blew out and blew bits of coal at the employee. He was struck in the face, neck and shoulder area by the bits of coal. Employee received minor contusions on the areas that were struck by the coal bits.

Employee was assisting in the replacement of a bearing on the transfer conveyor belt. Employee was checking the hole alignment of the bearing block to the conveyor frame by inserting his finger into the hole in the bearing block. However, the block shifted and pinched his finger, removing his nail in the process.
