Employee was clearing rock from crusher when they felt a tweak in their shoulder. It was initially treated with first aid. When the condition didn't improve, the employee received an MRI. The event became reportable on 2/9/2022 when a doctor placed EE on total disability following the results of the MRI.

Injured employee examined what needed to be cleaned on the transfer screen to move the gates. EE was in a squatting position and barred the material that was stopping the gates from moving. In the squatting position EE grabbed the chute opening with hands out and when EE pulled self up EE felt a sharp pain in back and a popping sound.

Employee received an open fracture on the tip of his left index finger while placing the main shaft sleeve on the crusher. His finger was caught between the main shaft sleeve and the mantle of the crusher when the lifting eye was removed before the sleeve was set in place and employee had not removed his finger. He required 3 stitches on the tip of his finger.

IE and another EE were replacing the Media Recovery Box for the Heavy Media plant. As they were removing the part, one side slipped catching IE's left index finger between the part and the screen deck. IE received 3 stitches and a fracture of the left finger.

Employee and co-worker were moving steel bars onto the fork of a telelift. One bar slipped catching the employee's glove on a piece of broken weld causing his left thumb to be caught between 2 bars. Employee sustained a deep cut and a bone chipped in his left requiring 5 stitches, splint and medication.

The employee was changing out a screen from a manbasket, when they tried to remove the screen over a bar, which caused a strain to the left shoulder.

EE reported to his foreman that he injured himself while cleaning out the east side crusher sometime in October 2011. He indicated that he understands that he violated company policy for not informing his supervisor at the time of the injury.

Plant Operator strained his low back on Monday, 12/06/10, while shoveling sand. On Friday, 12/10/10, the Plant Operator was carrying a 20 lb Come-Along up some stairs causing further strain to his low back. On Monday, 12/13/10, the employee reported the injury to his supervisor and that he was suffering from severe pain in his low back.
