Caught in, under or between a moving and a stationary object

IE and another EE were replacing the Media Recovery Box for the Heavy Media plant. As they were removing the part, one side slipped catching IE's left index finger between the part and the screen deck. IE received 3 stitches and a fracture of the left finger.

EE reported to his foreman that he injured himself while cleaning out the east side crusher sometime in October 2011. He indicated that he understands that he violated company policy for not informing his supervisor at the time of the injury.

Over-exertion in lifting objects

Plant Operator strained his low back on Monday, 12/06/10, while shoveling sand. On Friday, 12/10/10, the Plant Operator was carrying a 20 lb Come-Along up some stairs causing further strain to his low back. On Monday, 12/13/10, the employee reported the injury to his supervisor and that he was suffering from severe pain in his low back.

Over-exertion (Not Elsewhere Classified)

Employee reported that on 7/19, as he was attaching a water hose to a water line, he felt a pain in his lower back. He did not report it to his supervisor until the next day, 7/20 after clocking in. He was seen by the local clinic which took him off work for a back and hip strain. His lost time began on 7/21.

Struck by falling object

A small rock fell from from an elevated work platform striking the injured employee's right eye. The incident resulted in a scratch on the employee's right eye.

Employee reported that the front-end loader he was operating shifted to one side when it hit some debris in a roadway. The motion of the loader cause a twinge in his right hip area.

During normal operation of his haul truck, the employee reported that the bouncing of the truck and occasionally the bumping during loading caused a back strain.

EE HAD BEEN DIAGNOSED W/CARPAL TUNNEL & REPORTED IT ON 1/28/00. HE WAS PERFORMING SHOVELING AND OILING DUTIES WHEN HE FELT PAIN IN BOTH HANDS. EE WAS SCHEDULED FOR SURGERY ON 4/28/00 BUT LEFT PLANT ON 4/19/00.
