The victim was retrieving a lifting strap from the tool compartment of the truck. A tarp was partially covering the strap and EE gave it a yank to free it. It was tangled with a sheave (pully assembly). The sheave fell out of the tool compartment striking EE's right foot just behind the steel toe covering. Crushing injury to great right toe. No fractures detected.

Showing all 6Driver was attempting to raise the hood on coal truck. As driver was lifting hood from left side it caught on something. When it suddenly stopped driver had immediate pain to left shoulder.
EE was conducting pre-shift exam on highwall miner when ee closed the operators compartment door and caught the tip of right index finger in the latch mechanism. The tip of finger was amputated but was reattached.
Victim was making repairs on a 992-G loader. As he was dismounting a tool caught on the rail and made contact with his hand. Resulted in a laceration that required 10 stitches in his right index finger.
While working as a Greaser, tossed brake drum and something flew into his right eye.
While cleaning up maintenance yard, Injury lifted a piece of metal and felt pain in his groin area. No medical treatment or missed time. It was reported as an incident only. However, it appears that a hernia has resulted in this strain and he just sought medical treatment on 10/3/09.
