Employee stated they were operating a 785 rock truck when they ran through a deep hole. This caused them to strike their right knee on the gearbox, resulting in a contusion. The employee initially returned to work, but on 4/6/26, they returned to the doctor, who imposed restrictions. Employee is currently working within those restrictions.

Employee stated that while attempting to drain oil out of the 34103-rock truck, EE stepped into deep mud and fouled EE's foot. When EE attempted to pull free from the mud, EE heard EE's knee pop. Employee was treated by an orthopedic surgeon and taken off work pending surgery. Medical data was made available to the company on 3/31/26 that Part 50 requires to report.
Employee was walking down a slight grade when EE slipped and fell, injuring EE's right shoulder. EE returned to work the next scheduled shift, under restrictions for 2 weeks. This is reportable due to EE's job change for the 2 weeks of restrictions.

While descending ladder on loader, employee misplaced their foot onto the outside bracket of a step; leading to rolling of the ankle and then falling to ground.

Mechanic was using an wobble socket to take a bolt out of a CAT 777D transmission when the socket broke apart causing a avulsion on the skin on the left ring finger.
HWM forklift operator had the water tank on the forks. In the process of going to the water tree to fill it up, EE struck a rock in the road causing EE's seat to bottom out giving EE a sharp pain in their neck and back.

The tech and tech helper had removed a 650K Dozer roller frame from the machine and it on the ground. The roller frame had to be turned over to repair bottom rollers. The tech and helper used 2 slate/pry bars to roll the roller frame over. As the roller frame rolled over the slate/pry bar struck the tech on the left face area near the jaw causing a 2"" laceration.

The employee was dismounting the fork lift which is used to move and position the highwall miner cars when EE's foot slipped and EE dropped to the ground. EE landed on EE's right foot and immediately EE's right leg began to hurt mostly in the knee region,
The employee was attempting to pry an auger in the highwall miner car into alignment with the next beam (highwall miner car)that was being set for connecting. EE was using a slate bar and sustained a strain to the groin area due to over exertion.

The track slipped on excavator and operator tried to force it back on using the bucket. Victim was struck on the cheek by something. Initial thoughts were it was a rock but a follow-up exam showed 3 chips of metal in cheek. Outpatient surgery on 10/2/17 removed the chips.
The employee was using a utility knife to cut zip ties when the knife slipped and lacerated the left arm. Three (3) stitches were used to close the wound.

EE was conducting pre-shift exam on highwall miner when ee closed the operators compartment door and caught the tip of right index finger in the latch mechanism. The tip of finger was amputated but was reattached.
Employee was utilizing a Cat 980-G loader to stockpile coal as it came from the highwall miner. EE backed off the stockpile too fast and when EE contacted the pit floor it caused EE's neck to be jerked. EE states EE heard neck 'pop' and it began to hurt. EE finished shift, went to the doctor the next morning then reported for work.
We were notified on 1/28/2016 that EE had been ruled compensable for a black lung claim.
Victim walked over a bank & picked up water hose. As EE was coming up the embankment another man opened the valve. The pressure caused the hose to jerk EE around and fell over the bank. Tore left bicep.

Victim was making repairs on a 992-G loader. As he was dismounting a tool caught on the rail and made contact with his hand. Resulted in a laceration that required 10 stitches in his right index finger.
Victim states he was loading shot rock into truck when a rock slipped and rolled back onto the loader bucket causing his back to be injured.
Victim was loading shot spoil into trucks when he struck a large rock in the spoil. The sudden stop jerked him forward into the seatbelt causing back pain and also jammed his right knee into the control box injuring his knee. Further tests are scheduled to determine extent of injury to his knee.

Employee was attempting to dislodge a stuck conveyor on the highwall miner with a 5 foot pry bar when the bar slipped and struck him in the left jaw, leaving two lacerations. Employee chose not to receive medical attention at time of accident and finished the shift. Employee later decided to get medical attention, at witch time he did receive stitches to close the lacerations.
Employee was stepping up on the 777 truck #4012 when he slipped on step. Employee claims to have strained lower back and right leg. He reported both are sore.
5:50 pm security Officer noticed black smoke coming down valley, went to check it out. Found 992G Loader on fire. 6:06 pm called person from Raven Crest. 6:08 pm called 911 at time of arrival on scene fire was coming out around both back tires and up over the top of them. Didn't see any one around, Security Officer went back and waited at post 4 on fire trucks.

Operator was driving from the loading point to dump site when the front right wheel dropped into a low spot on the haul road causing operator to lunge forward and to the right. Seat belt stopped the operator and caused a sharp pain in his back and shoulder. Transported to the hospital for evaluation.

EE had just completed setting up water pump. After it began running, rock fell from wall and landed on 30' bench spraying out to level EE was on, striking him and the pump.

EE was driving a 777 CAT rock truck, while a loader operator in a 992G was was loading him. A rock came out of the loader bucket and hit the off side of the rock truck bed, jarring him and causing him to go side to side in cab of rock truck.
Sitting on ground, reached up with left arm to pull self up, lost balance fell with full weight on left shoulder. Completely dislocated left shoulder.
Backed over a big rock, hit hard and jarred lower back - did not seek medical treatment until 3/2/07.

EE was loading a big rock into a 777 hauler when the rock came out of his bucket, jarring him. EE complained of back pain and left early, but returned the following shift and worked out the week. Over the weekend EE said his back worsened. He began missing work on Monday July 24,2006.
Employee was climbing up the ladder of a 785 Hauler when the weld on the bottom three steps broke, causing him to lose his grip and fall to the ground. employee landed on his lower back.

Employee was operating a 777D rock truck when a loader put a large rock in the bed of his truck. The rock was off center, which caused the hauler to rock sideways. EE hit his left shoulder on the door post.
Employee was mounting a 988G Coal Loader when his hands slipped off the hand rails causing him to fall off the loader. He did not receive medical care or miss any time until November 6, 2005.
