Employee was lifting a diesel particulate filter when EE felt pain in EE's right shoulder. A later MRI revealed a tear in EE's rotator cuff. EE continued to work until 1/14/25 when EE had surgery to repair the tear and was taken off work as EE was unable to perform EE's job without restriction.
While lifting the framing at a header with right arm the belt foreman felt a burning sensation in right shoulder. Worked until surgery date. Started missing on 03/25/2026.
Employee was drilling a hole when steels became stuck. As they pulled them apart, one struck them in the nose. Employee was seen at the ER where x-rays showed a fracture. Employee was given a full release and continued working until 11/12/25 when seen again and given restrictions preventing them from performing their normal job.
Employee was using their foot to slide boards under the tongue of an RA and when they put their weight on their right leg, they felt their right knee pop. Continued working until 12/1/25 when taken off for surgery to repair their right knee.
The miner operator used EE's index finger to rub EE's right eye causing dirt from EE's glove to enter and scratch the eyeball. Seen the doctor on 10/15/2025 where medication eye drops were prescribed and no work was missed.
Employee was operating a grease gun injecting grease into a fitting on the continuous miner to tighten the cats. The connection was stuck, while prying on the connection, it popped off and caused a laceration to EE's forearm. EE received sutures to EE's forearm, was given a full release without restrictions and performed EE's normal occupation.

Employee continued to work without restrictions performing EE's normal task, and was seen by a doctor and was taken off work on 5/21/2025 as EE would not be able to perform EE's normal work duties
While cleaning the scalping screens on the 3rd floor there was some gob that broke loose in the chute just above and hit the utility man in the top of the head. Continued to work until 12/11/2025 when surgery was required to repair EE's neck.
While picking up a trailer hitch to install on a tractor the employee strained their lower back. Started missing work on 04/29/2025.
Employee was cleaning out around a clamp on a waterline and cut EE's right index finger on a sharp rock. Glued the finger and no work was missed.

EE was reaching for a second steel and when EE turned back around the first steel fell out of the mine roof and bounced off the drill pot knocking EE's glasses off causing the steel to hit EE on the left eye. EE was seen by a doctor and was given eye drops and a protective contact lens.
