Employee was lifting a wire saw tree with it rested on employee's arms and hurt left bicep. Employee called the following day and said employee needed a day to rest the arm.

Showing all 15Pulling an oversized tarp to fold it up. Drove to portal 5 and when they got out of the truck they felt a sharp pain on the right side of left knee.
Employee was securing water pipe to roof when tension on the pipe released pushing against shoulder which caused pain.
Employee was measuring the depth of a saw cut and got EE's fingers caught between the marble and a piece of rebar pinching EE's fingers.

Employee was sitting between the blocks while one was being trimmed. The trim piece separated and fell on his legs. There was a ladder lying by him. Co-worker had the guys bring a piece of steel to try and take the pressure off his legs. They brought in a loader and picked up the edge of the block a few inches so they could get his legs out.
Blowing out water line and the pressure knocked off his safety glasses and he got something in his eye.
Employee was kicking diamond wire to free it from the ice and hurt his foot.
Employee was moving his saw when the guard fell down hitting him on the head. Employee said his neck was sore and he had a headache. Employee was taking Advil and refused to go to the doctor. On June 2 employee said when he stopped taking the pain medication his neck hurt. We made him an appointment to go to the doctor.
EE was holding a ladder for another ee to the side of the loader forks when a rock fell off of the forks hitting ee in the right pointer finger and right leg.
EE was cleaning out mud out of the cut and a piece of marble fell about ten feet striking him on his hardhat.
Employee was lifting a tire chain to hang it on the wall. It was at the end of his shift, but he did not report it to anyone.
July 2, EE was on E floor watching wire saw. Employees on top of C wall were trying to clean up for a spot to drill. A 4'x4' piece of wire barrier fencing came loose and fell from the upper level, striking EE on the hard hat, knocking him to the floor. Created intense headache - he went home because shift was over.
Employee was attempting to unload 400 lb equipment out of the bucket of the loader. As the Hotsey (equipment) was coming out of the bucket, employee slipped on the ice, and the equipment landed on left foot, pinning it to the floor. Got boot off, could see indention and bruise. Concerned about fracture.
Sore back. Employee indicates he strained his back on 4/21 while working on the skidsteer. In later conversation employee indicates he strained his back while assisting in stringing a wire saw cut. Did not report the injury until 1:00 p.m. April 22, 2008.
