Struck by flying object

Employee was adjusting valves on the bag pump when the hose popped off of the fitting. The water blew EE's safety glasses off and the hose hit EE near EE's eye. Employee stated several days later the eye still hurt and EE went to the hospital.

Over-exertion in lifting objects

Employee was lifting a wire saw tree with it rested on employee's arms and hurt left bicep. Employee called the following day and said employee needed a day to rest the arm.

Fall from ladders

Employee was coming down from a ladder and missed the last step. When they stepped down to the ground, they rolled their ankle slightly. Employee stated that they did not think it was an issue and filed a report when they could not put much weight on it the next day.

Pulling an oversized tarp to fold it up. Drove to portal 5 and when they got out of the truck they felt a sharp pain on the right side of left knee.

Employee was securing water pipe to roof when tension on the pipe released pushing against shoulder which caused pain.

Struck by flying object

Employee left a wrench on a drill and forgot about it. Employee then turned the machine on causing the wrench to spin and come off and hit the employee in the ankle.

Employee was walking over to see another employee and fell forward breaking both arms.

Employee was measuring the depth of a saw cut and got EE's fingers caught between the marble and a piece of rebar pinching EE's fingers.

Struck by falling object

IE and another EE were removing the steels from a Fast 65 hole. The bottom steel didn't come loose and when it turned the wrench hit IE in the left ankle.

Cut/crushed left pointer finger picking up a bar.

Employee was sitting between the blocks while one was being trimmed. The trim piece separated and fell on his legs. There was a ladder lying by him. Co-worker had the guys bring a piece of steel to try and take the pressure off his legs. They brought in a loader and picked up the edge of the block a few inches so they could get his legs out.

Employee was removing a pin from a wire saw when another employee hit it with a sledgehammer. The pin came loose and hit him in the right knee.

EE was setting up the wire saw and went to show another ee the controls. He tripped on the diamond wire and hurt his left wrist.

Blowing out water line and the pressure knocked off his safety glasses and he got something in his eye.

Employee was getting his lunch box out of the pickup truck and got his foot stuck in the mud. When he went to leave he twisted his right knee.

Employee was moving his saw when the guard fell down hitting him on the head. Employee said his neck was sore and he had a headache. Employee was taking Advil and refused to go to the doctor. On June 2 employee said when he stopped taking the pain medication his neck hurt. We made him an appointment to go to the doctor.

EE was holding a ladder for another ee to the side of the loader forks when a rock fell off of the forks hitting ee in the right pointer finger and right leg.

Employee was walking around the connex area when a rock rolled out from under his foot. He fell and hurt his lower back.

EE pulled a pump off of a block and slipped on the ice and fell and hurt his back.

Employee was walking outside the mine and stepped in some mud and pulled his foot out and twisted his knee.

EE was moving wire when another employee attempted to start the wire saw twisting EE's finger. EE said the accident happened on 9-16-2013 but did not report the accident to anyone until 12-16-2013, 3 months later. He was at work everyday since 9-16-2013 and continues to work up to the time of this report. EE went to the doctor on and they said he might need surgery.

EE was using a jackhammer when the jackhammer spun around and the handle hit EE in the chin.

EE was cleaning out mud out of the cut and a piece of marble fell about ten feet striking him on his hardhat.

Employee was lifting a tire chain to hang it on the wall. It was at the end of his shift, but he did not report it to anyone.

Fall from ladders

Ladder was leaned against a 20 ft wall. Ladder should have been leveled and tied off before worker attempted a small drill hole near the top. Ladder slipped, and worker fell. Broken both wrists/arm, finger, impact left elbow, left knee, left shoulder, and ribs. Face injury, teeth.

Employee reported this injury at least one month late, continued to work. Mentioned headaches, co. immed. scheduled Dr. apt. He fell 8 ft off ladder, in mid/late January. Descending a ladder, bottom slipped, fell to the floor. Got up, said he was fine, continued working. Four Witnesses.

Employee was lifting a Korfman chain saw with a hydraulic jack to level it for a cut. He had the jack set in 2 inches of mud, and the jack tilted to one side when it took on the weight of the saw. The machine came toward the wall and pinned him at the knee. Injured left knee, chipped small piece of bone on patella, bruised knee area.

Outside of quarry checking discharge water line. Stepped off a rock about 18 inches high into snow-covered area. Stepped on rock, twisted left ankle, resulting in strain and fracture.

Bobcat operator appeared to be having difficulty getting back up the ramp out of the way of the Loader. Loader operator was going to assist, Bobcat operator put Bobcat in reverse, making the Bobcat jerk. Jarred back of Bobcat operator.

Struck by falling object

July 2, EE was on E floor watching wire saw. Employees on top of C wall were trying to clean up for a spot to drill. A 4'x4' piece of wire barrier fencing came loose and fell from the upper level, striking EE on the hard hat, knocking him to the floor. Created intense headache - he went home because shift was over.

Struck by flying object

Pulling a block out from wall with Kamatsu PC400 track hoe and cable. The shackle broke and 1 piece came through windshield, hitting operator in the head and went on to hit the back window.

Working on floor, tripped on something (wood or rock) fell and landed hitting his right hand on a sharp rock.

Employee was attempting to unload 400 lb equipment out of the bucket of the loader. As the Hotsey (equipment) was coming out of the bucket, employee slipped on the ice, and the equipment landed on left foot, pinning it to the floor. Got boot off, could see indention and bruise. Concerned about fracture.

Over-exertion in pulling or pushing objects

Sore back. Employee indicates he strained his back on 4/21 while working on the skidsteer. In later conversation employee indicates he strained his back while assisting in stringing a wire saw cut. Did not report the injury until 1:00 p.m. April 22, 2008.

Employee was holding a wrench to loosen the drill steel from hammer head, when rotation started, pinning his thumb between the wrench and frame of machine. His left thumb had a crushing type injury.

Over-exertion in pulling or pushing objects

Working on Fantini saw tracks, replacing leveling pads. One pad very stuck, left shoulder had major pain. Iced, no treatment by doctor. Employee reported to me 5/27 that he had gone in to doctor for pain and given a cortisone shot. Awaiting decision regarding surgery.

Reaching over edge of a block for a T-square, lost his balance falling approx 5 ft. Landed in some rubble on his right ankle and leg.

Attempting to lift a truck tire edge so he could get the skidsteer bucket under it to move it to another location.

Moving wire saw from an area with the Bobcat, bucket of Bobcat was up to the ceiling of the area and broke loose a piece of thin marble which fell and hit employee's hardhat, breaking into smaller pieces, hitting his cheek and left hand.

Attempting to straighten a 12"" anchor pin with drill press. Pin popped out of hydraulic press and hit his right hand - 5 stitches.

Employee walked across trash and broken stone area to change compressor setting. Large rock was dislodged and trapped his left foot (fracture). Should have walked around the area and used ladder provided.

Thawing mud and ice conditions at top of wall. Miner stepped off ladder to edge of wall. The frozen mud gave way and he slipped, falling 8-1/2 ft backward. Landed on right side/knee.

Contact with electrical current

Got a shock from 440v cable short when he grabbed diamond wire on saw.

Ingestion of radiations, caustics, toxic and noxious substances

Transferring diesel fuel from transportable tank to generator tank, fueling hose was inhibiting operation of crank-style pump. Employee moved hose to allow cranking, hose slipped off nozzel, spraying fuel at other employee in face, eyes, front, causing ingestion of fuel. Immediate vomiting. Employee was sick 2 hours, finished shift to 10:00 p.m.

Working underneath Track Hoe and some dirt or debris got into his eye as he was brushing off his hair and face after finishing. Was wearing safety glasses during the work, removed them to brush dirt from hair and face.

Miner was hitting a wheel with a 3 lb hammer to adjust the angle. Missed and hit is own right hand. Bruised right hand. Was x-rayed, no broken bones.

Struck against a moving object

As the crew was riding in the Suburban vehicle on the way to the Quarry, they crossed a rough spot, caused by snow melt run-off in the road, slowly. The bounce of the vehicle caused employee to jam his neck on the ceiling. He was in the back seat area.

Struck by flying object

Drilling with hand drill when the fitting on the drill came off and hit the back of his hand. Bad bruise on right thumb and back of hand. Employee had performed all safety precautions.

Employee moving a drill of approximately 100 lbs, and feels she strained her shoulder and arm when she lowered it to the ground from an upright position to the ground

Miner was decending on ladder tied off ladder. Ladder slipped, miner lost balance and fell approx. 8 ft. Hit right shoulder and head on floor. Iced/heat applied to shoulder and neck immediately. Headache and stiffness in shoulder/neck area.

EE WAS STEPPING OFF CART ONTO UNEVEN GROUND AND SLIPPED. KNEE TWISTED AND SHE FELL, LANDING ON HER WRIST ON THE THE GROUND.

EE HAD FORGOTTEN TO PUT AWAY THE CRANK HANDLE ADUSTER FROM PREVIOUS CUT. WHILE MOVING CART TO SET UP FOR NEXT CUT HE WAS HIT ON THE INSIDE OF HIS KNEE BY THE SPINNING HANDLE.
