Showing all 8While working on 14 Belt head drive, a concrete block, which was secured to the roof by plaster, let loose, striking him on his lower right arm.
While pushing a roof bolt up to the roof, the employee thought he had the bolt head in the bolter chuck. However, the bolt was not in the chuck, which allowed it to slip, striking the employee in his right forearm.
While pulling the belt together, the belt clamp slipped, and the clamp struck him on the left forearm.
