Employee was helping a truck driver get a rock out of the tailgate of their dump truck. As employee and truck driver pulled the tailgate open, the rock fell out, truck driver let go of tailgate, the tailgate swung shut and struck the employee's right pointer finger. Employee's right pointer finger was fractured.

Showing all 5Employee was removing 4" discharge pipe from the CFS slurry pump. After removing the pipe, the pressure that had been built up inside of the pipe let loose and hit the employee in the face with sand and water. Employee suffered burns on both cheek bones and above left eye. EE has safety glasses, hard hat and ear plugs on.
