Employee was sitting inside of a skid steer. When the employee climbed out of the skid steer, EE slipped on the ground and twisted EE's left knee.
Employee's yearly hearing test showed a standard threshold shift.
Employee was walking across parking lot when he slipped on ice and fell onto his back resulting in a contusion to his chest wall and back.
Employee was installing blade guards on a P65 Dozer for transport when his little finger on his right hand was pinched between the guard and the track frame causing a laceration requiring 4 stitches.
Cleaning the cone crusher with a pry bar and his finger was squeezed between the bar and guard on the cone crusher. He lost his nail and received 17 stitches.
A PIECE OF STEEL SLID DOWN HITTING THE EMPLOYEE ON THE RIGHT EAR WHICH REQUIRED 10 STITCHES TO THE EAR LOBE.
