Employee was removing a tail pulley guard, when it slipped. The employee felt a pop in their arm. The employee did not report the incident to management for 3 days. The employee then sought medical treatment and was placed on restricted work duties for a pinched nerve.

EE opened the door to the rock chute to start dislodging the materials, while placing the right hand on the door opening, when a rock fell loose, striking the EE on the right hand.
Sand and water caused pressure washer wand to slip from my hand and injected water under my skin. Minor impairment.
Employee was unloading some pipe from a truck. He picked up the pipe to load the bucket. When he picked up the pipe he felt a twinge and pain in lower back and left leg. Employee was on restrictive duty and as of 3/2/16 he is a Lost time. This incident was not reported at the time due to improper communication. This incident is reported late.
Employee was lifting a screen (less than 100lbs). Received help from another. After lifting the employee felt a sprain/strain in left arm. Employee was taken to the local clinic where they put on restrictive duty with a sling. Going back for checkup to re-evaluate.
IE and another EE were lifting a small hopper weighing about 125 pounds onto the tail pulley of the concrete sand stacker. One of the hopper legs struck IE on the left leg behind the knee, causing a deep laceration.
