Fall onto or against objects

Employee was climbing up a Volvo A35G Haul Truck when they lost their footing and fell forward into the battery box on the truck hitting the bridge of their nose. The employee sought medical treatment and received stitches. They were released to full duty on the same day.

Employee was shoveling underneath a tail pulley. They turned with a full shovel and experienced pain in their upper and lower back. Employee was placed on restricted duties 6 days later.

Struck by rolling or sliding object

An employee was clearing the rock box when a lodged rock rolled loose and struck the employee's finger. The employee was diagnosed with a fractured finger and placed on restricted work duties.

EE opened the door to the rock chute to start dislodging the materials, while placing the right hand on the door opening, when a rock fell loose, striking the EE on the right hand.

Team member was washing material off the concrete pad when he slipped and fell onto his left knee. He declined immediate medical attention. On November 18, 2013 the doctor diagnosed his knee as a sprain and employee returned to work on November 18, 2013 on restricted duty.

Employee was attempting to roll a rock out from the crusher and in the process strained his lower back.

EE had been in a loader loading trucks and came to his supervisor complaining that something came off his shirt and got into his left eye. He was then told to go get medical attention.

EE was replacing a casing in a pump and while lifting the pump casing the cable rachet puller broke, causing the pump casing the fall and strike the employee on the right ankle.

Employee was driving a loader behind the impact crusher to check the belts and in the process a rock was thrown from the crusher hitting the door glass and causing glass to break inside the loader. He got a piece of glass in his left eye.

EE WAS EXTRACTING RAW MATERIAL FROM THE NORTH WALL OF THE [PIT AND LOADING THE RAW MATERIAL ONTO THE TRUCK THAT COWORKER OPERATED. EE POSITIONED THE EXCAVATOR PARALLEL W/ THE NORTH WALL. AT ~1 415 COWRKER RETURNED FROM THE STOCKPILE AREA TO THE PIT AREA & NOTCIED THAT THE NORTH WALL HAD COLLAPSED & FALLEN IN ON THE EXCAVATOR. CALLED 911 & WENT TO RESCUE EE. EE WAS RESCUED & TAKEN T

EE WAS WATCHING AN EMPLOYEE MOVE A METAL CONTAINER AND IN THE PROCESS THE EMPLOYEE TURNED THE CONTAINER UP ON ITS SIDE CAUSING THE CONTAINER TO FLIP OVER. EE WAS UNABLE TO MOVE OUT OF TH EWAY WHEN HE REALIZED THE EMPLOYEE DID NOT HAVE CONTROL OF THE CONTAINER AS HE TURNED IT UP ONTO ITS SIDE. THE CONTAINER THEN FELL ONTO EE'S TOE.
