Showing all 33Was parking block at '14450 in between entries #2 & #3, a truss bolt broke, the strap came down hitting him on the hard hat sliding down to his right wrist. He was seen at the hospital and released to work. On 6/19/2015 he was placed on restricted duty.
Was hand dragging a bolter cable down the #1 entry in 3-Left, he had the bolter cable over his right shoulder when the cable got hung on a rock, he felt his right shoulder pop.

On a bolter he had the rub-rail off the operator's side of the mass, working on the suction hose. He went to put it back on and felt a pop in his groin area.
Was roof bolting when the lower drill steel became stuck in the drill pot. While trying to remove the steel from the drill pot, an upper section of the drill steel fell from the bolt hole, striking the employees right hand.
Was running jumper hoses from the pump car to the monorail. Was dragging a 50 ft. section when he collapsed with back pain.

When opening the door of the Sandvic LS195 while still in the cab. His hand was caught between the cage on the door and the cage on the frame; causing his finger when opening the door to be smashed between the door and the frame.
He was standing a 25 stack of hog panel up when the previous stack became dislodged. Falling onto the employee striking him in the face.
Employee just stepped through an approximate 3' x 3' mandoor at xc22, the air caught the door and slammed it shut abruptly. Employee was holding onto the door handle with his right hand, as the door slammed shut, he pulled his hand away quickly and his index finger was struck by the door or he struck his hand on the door frame.
He was standing behind a Kubota putting a tire in the bed, the tire caught on the corner & he felt a pop. We received a note from the doctor to place him on restricted duty on Oct. 1, 2014

While loading a structure trailer into the belt-line, a crew member grabbed a rail and started to load, as he jerked the rail out he struck ee in the nose.

Employee was moving continuous miner cable out of the entry in preparation for mining. When he lifted the cable to his waist he felt a sharp pain between his shoulder blades and shot down his back. He went down on the ground and had lost the feeling from his shoulders down. No physical damage has been identified by the medical testing.
Employee was using a roof bolting machine to install chain hangers in preparation for a belt move. While pushing a steel back into the drill hole, the steel suddenly angled off to one side. As the employee reached to straighten the steel it recoiled back, striking his left index finger. The impact fractured the finger.
Employee was shimming up under a small column held in place with a chain fall and it shifting slightly, catching his right hand in a pinch point. The employee was wearing metacarpal gloves at the time.
Employee was operating a roof bolter, installing wire mesh on the corner of a pillar. He leaned over to pick up a section of mesh from the floor, while on the drill deck and pulled a muscle in his back. He sought medical attention on 9/16/2013. Medical provider placed employee on restricted duty. Employee returned to work under these restrictions.
Two employees were moving top roller assemblies up against the rib. They had moved several without incident. While moving the roller, the employee felt a pop in his lower back resulting in a back strain injury.

Person had strained a muscle in his back off the job at home. He re-strained the muscle hooking up a tow cable at work.
