Walking from mantrip, he stepped on a chunk of rock and twisted left leg and knee.
Roof fall at #2 entry, xc #89 on the 3-Left Headgate. 8' TR bolts with 12' cable bolts in intersection. Fall did not cause injury, property damage, affect ventilation, or impede travel.
While changing hinge pins and bushings on the CM14-27, a mechanic was prying up on a cover and hoses, the pry bar slipped and hit him in the face.
He was bolting in low top, he was pushing up a bolt with his left hand to get on the pod to push up and strained his left shoulder.
Was parking block at '14450 in between entries #2 & #3, a truss bolt broke, the strap came down hitting him on the hard hat sliding down to his right wrist. He was seen at the hospital and released to work. On 6/19/2015 he was placed on restricted duty.
Was hand dragging a bolter cable down the #1 entry in 3-Left, he had the bolter cable over his right shoulder when the cable got hung on a rock, he felt his right shoulder pop.
Was spotting a 3" bolt in slate. Started drilling, the rock broke into pieces. He stepped back on platform, and a piece of the rock caught his hand against the rear of the back rest on the platform cutting his hand.
On 4/26/2015 while examining the SCBA's on the Main North, he tripped over a cable bolt and fell on his left arm. He was taken off work in 8/5/2015 for surgery on shoulder.
Was standing up hog panel and strained his abdomen.
A miner was cutting in entry, employee was operating a shuttle car behind the miner loading, a piece of carbide flew off and struck him above the eye.
While drilling a 6' hole, he drilled the top steel in the roof and reached for the bottom steel, the top steel slipped out of the hole striking in on the middle finger.
Employee was putting floor grading at the belt drive, a strap fell between the grading and frame and when he reached for it he pinched his finger between the grading and frame. (Fractured left index finger)
While changing out batteries he took hold of the jumper cable causing him to be shocked by the cable.
Pulling pads together on a single boom bolter, while tightening a come-along the hook came loose from the cat pad and the hook struck him in the right eye. Suffered a laceration to the eyelid and laceration to the cornea.
Unplanned Roof fall at 5 MN xc25. Fall occurred in intersection (4-way)slip plane that was supported with 72" TR bolts and 11 ft. cable bolts. fell from roof. Fall dimensions 19'W x 30'L x 8'-10'H. Fall did not cause injury, impair ventilation, or entrap miners.
Employee reported on 2/4/2015 that on 1/16/2015 he bumped his left knee against a knob on the side of a scoop.
crew was off loading 42" structure when he caught his finger underneath the bottom roller causing a laceration. (required stitches)
Employee caught his foot on a pedal as he was getting out of Sandvik, fell out of the door to the ground. He attempted to catch himself with his left hand and injured it.
When prying shuttle car tire off of a broken wheel unit, the tire came off and smashed his finger between the slate bar and coal rib.
On a bolter he had the rub-rail off the operator's side of the mass, working on the suction hose. He went to put it back on and felt a pop in his groin area.
A rock fell out from the roof between mesh sheets while installing a roof bolt, cutting his left arm.
Was roof bolting when the lower drill steel became stuck in the drill pot. While trying to remove the steel from the drill pot, an upper section of the drill steel fell from the bolt hole, striking the employees right hand.
Was closing the lid on a battery powered scoop, when a rock fell between the bolts, striking the employee in the back of the head.
Was roof bolting when a rock fell from the roof, striking the employees right hand near his thumb.
Unintentional roof fall above bolt anchorage in Main South entry #7 between crosscuts 100 and 200. There were no injuries, and travel/ventilation were not affected.
He was walking down the panline, slipped and twisted his right knee.
Carrying and dragging chain & foundry hooks from the Tailgate to the Headgate. He fell landing on his shoulder.
Hoist controls faulted. En coder fault and communication cable problem.
While turning to walk away from stopping, he slipped on hard surface covered in mud. As he was falling his leg twisted beneath him and he heard a crack.
Was running jumper hoses from the pump car to the monorail. Was dragging a 50 ft. section when he collapsed with back pain.
Was pulling cables through trollies, felt pain in abdomen
When opening the door of the Sandvic LS195 while still in the cab. His hand was caught between the cage on the door and the cage on the frame; causing his finger when opening the door to be smashed between the door and the frame.
He was standing a 25 stack of hog panel up when the previous stack became dislodged. Falling onto the employee striking him in the face.
Employee just stepped through an approximate 3' x 3' mandoor at xc22, the air caught the door and slammed it shut abruptly. Employee was holding onto the door handle with his right hand, as the door slammed shut, he pulled his hand away quickly and his index finger was struck by the door or he struck his hand on the door frame.
Injured employee stated he was helping move a belt pulley over top an overcast when the top of the overcast collapsed and he fell to the ground striking his head.
Roof fall at #2 heading, 496.5' station. Fall above anchorage zone of bolts. 8' TR bolts w. 12' cable bolts in XC. Fall did not cause injury, property damage, affect ventilation, or impede travel.
He was standing behind a Kubota putting a tire in the bed, the tire caught on the corner & he felt a pop. We received a note from the doctor to place him on restricted duty on Oct. 1, 2014
He was installing 16' cable bolts, 4 sections of drill steels being used. 3 sections had been removed from hole, and the top section fell from hole, striking his left hand (finger) between the steel & the drill pot.
Was pulling out a VFD inverters out of cabinet to clean and the inverter fell out and cutting employee's finger

While lifting the hitch on a trailer, strained her back.
While loading a structure trailer into the belt-line, a crew member grabbed a rail and started to load, as he jerked the rail out he struck ee in the nose.
Operator was standing on platform drilling corner rib bolt. Steel clogged up & the operator was using a wrench to beat steel to unclog. The wrench came off steel and struck his hand against corner. (Back of hand).
Miner was bending over to move miner cable & felt a sharp pain in his lower back.
Employee was bolting on the right side of bolter. While installing the last row of bolts, the face rolled out at the top. He was startled and jumped back to avoid being hit, fell backwards onto the bolter walkway.
employee was building brattice, while holding cap board in place with one hand and hammer in the other, he hit the tip of his left thumb with the hammer. (partial amputation of the tip of his thumb and nail bed laceration requiring 5 stitches)
Belt Move - was picking up top roller, stepped in a hole losing footing. Strained back.
Was loading hog panel onto bolter from side of machine. A stack fell against bolter catching his arm causing laceration.
Employee was moving continuous miner cable out of the entry in preparation for mining. When he lifted the cable to his waist he felt a sharp pain between his shoulder blades and shot down his back. He went down on the ground and had lost the feeling from his shoulders down. No physical damage has been identified by the medical testing.
Employee was operating the continuous miner in #8 entry in the North Mains. The rib on the right side of the entry came off and struck the employee, knocking her down and some material landed on her hips and lower leg.
