Employee was changing a gearmotor for the mill trommel. The gearmotor was located on the floor. The employee, along with one other employee, lifted the gearmotor from its mount and moved it out of the way to make room for the new gearmotor to be installed. The next morning, the employee reported lower back pain and was refered to seek medical care.

The worker was unspooling the cable and walking it back toward the load-out area to connect to a railcar. EE felt a pain in EE's lower back.

Employee was lifting fork of extended reach lift to adjust width of forks. EE dropped fork into place. EE's hand position allowed the tip of pinky finger of EE's right hand to be crushed between back of fork and support cross member of backstop.

Customer truck driver sustained minor burns to the hands and face and swallowed a small amount of antifreeze when checking the coolant levels on their truck, which was running hot.

Operator was doing the routine end of week equipment shutdown and clean out when he got a chemical into his eye. Investigation determine there was a small drip from the line one floor higher. Employee was wearing all required PPE including safety glasses.
