While retrieving an oil supply hose from a service island the hose became hung up on the outside of the reel causing the employee to stop short straining the right shoulder.

A strap was being used to check the pivot bearing on a grader. The strap broke and struck the employee in the left hand. Ligaments in the left hand were weakened and the employee was released on light duty.

Employee was working on the explosive crew and was carrying a 35 pound box of boosters across the pattern and felt a sharp pain in their right knee. The employee has managed the injury but required surgery on 1-29-2019 which is also the first missed shift for this employee.

As employee was climbing the boarding ladder on to 59 haul truck they contacted their knee with the boarding ladder attachment. Employee later called the supervisor reporting that their knee was injured and their neck was sore.

Employee was repairing exhaust on a D10T dozer and strained their right wrist. Physician released employee to work. Physician prescribed a support brace on 11-22-2017 and the wrist required surgery on 1-16-2018.

Employee was removing a carrier plate off of a D11 dozer and had their left thumb pinched between the plate and the swing frame. The employee pulled their thumb free and noticed it was bleeding. Local medical center doctor advised employee to come in after the shift. The wound took 2 stitches to close.

Employee was removing a wheel chock from the bumper of a haul truck and felt a pop in their right shoulder. Employee was seen at local medical center and diagnosed with non-surgical tears in the rotator cuff muscles.

Employee was assigned duties as a shop mechanic. Employee was installing a tire on a haul truck and claims that they injured their back while performing above mentioned duties. The Mine Operator is contesting that the claimed injury occurred on the mine site.

While moving chock blocks and pulling on a hose reel, an employee had a sharp abdominal pain. Employee was taken to the hospital for observation. It was determined the employee had a hernia that will require surgery.

Employee complained of back pain after sitting in a meeting. Employee took next scheduled shift off due to the back pain. Workers Comp accepted the claim on 3/22/2016. At this time the incident is under investigation and the company has objected to the claim.

An employee was working on a haul truck when dirt and rust debris fell making contact with their face. Some of the debris went between the safety glasses and face getting into the employees eye. The employee was transported to the hospital where a pysician was able to remove the material and an antibiotic was prescribed for the eye.

While EE was putting cutting edges on a grader the cutting edge fell off the magnet at approx. 3 feet high and struck the EE on the left foot. The cutting edge landed on the back side of the EEs steel toe boot causing a fracture in his left big toe and required stitches to his left big toe.

Employee was moving a metal plate and felt burning in his groin area. Initial treatment was ice and ibuprofen. Employee said he felt ok. Days later the employee had a doctor examination and it was determined a hernia that needed fixed. Surgery was done on 12/12/14.

An employee was pulling on a rail switch when they felt a burning sensation in their back. The employee woke up the next morning with numbness extending down the left leg. After the investigation and an MRI, it was determined that the employee had an acute herniation of the L3 and L4 vertebrae. This information was made available to this office on 10/14/14.

Employee was boarding their truck and struck their left knee on the deck above the ladder. No treatment was given until 10/29/14 when surgery was done for a torn meniscus.

An employee lifted a chock block to place it back on a fuel truck. This action caused him pain in his right arm. No medical treatment other than ibuprofen was given. The employee stated that the pain never went away and it was looked at by a doctor on 8/5/14. At this time the employee was given work restrictions and surgery was scheduled. Surgery was done on 8/13/14.

While attempting to close the hopper door on a train car, one of the employees feet slipped on the snowy ground causing his little finger to get caught in the hoppers locking mechanism. This caused the bone in the tip of the finger to be broken and caused a laceration requiring two stitches.

The employee was working on a booster pump. While pulling on a six inch pipe trying to get it into place, the employee felt a pop and tearing sensation in his left shoulder. This shoulder had been injured previously in 2010 that required surgery.

A bead flange from a haul truck tire fell from tire being installed landing on the left foot of employee breaking the employee's foot in multiple places. Employee required surgery.

Employee was removing an apron cylinder from a scraper with the overhead crane. The cylinder became stuck, employee wiggled the cylinder loose. When the cylinder came loose his right middle finger was pinched between cylinder and machine. Employee was taken to the Medical Center, finger was x-rayed and showed finger was broken.
