Contract employee was trying to set a poly line in a stand. When EE was trying to adjust the poly pipe EE pulled pipe over stand. It came off stand slide across manlift rail and hit EE in the ribs.

Showing all 4The plant was shut down for the day and the miner was inspecting the plant and went to inspect the crusher blow bars. The miner opened the inspection door on the crusher to check the blow bar condition. When moving the rotor to inspect the gap the miner's hand got caught between the rotor and the inspection door frame and bent EE's wrist back fracturing EE's wrist.
The mine EE was making visual checks of the plant feed hopper and identified the belt was slipping, EE went to apply belt dressing. Sand had built up on the head pulley & EE attempted to scrape the sand off the pulley, EE's arm got caught in the belt & pulley. The miner was able to pull arm out. The results were skin abrasions, torn away flesh, & a fracture to the right wrist.
While greasing bearings caught RH middle finger between pully and drive belt.
