The plant was shut down for the day and the miner was inspecting the plant and went to inspect the crusher blow bars. The miner opened the inspection door on the crusher to check the blow bar condition. When moving the rotor to inspect the gap the miner's hand got caught between the rotor and the inspection door frame and bent EE's wrist back fracturing EE's wrist.

The miner was installing brackets for teeth on a loader. The brackets were tight and a sledge hammer was used to get the bracket in place. When the miner made one of the swings with the hammer a piece of the bracket chipped off and lodged in the miners left thigh. The miner was taken for medical attention to get the piece removed and stitches were used to close the wound.
