Diagnosed as a abdominal strain (No hernia).

Employee was offloading cribs from the de-energized Mule to shield toes on the longwall tear down face. While performing this task, EE was bending, twisting and not using proper lifting techniques. Employee was attempting to remove a drill steel that was stuck in the mine roof. As the employee went to place another drill steel back in the chuck, the steel that was stuck, fell out striking the side of the employee's left index finger. Employee underwent surgery on 12/20/17 to repair a left bicep tendon. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.
