At approximately 4:20 PM the western slope of the Old Coarse Refuse was discovered to have 3-4 "hot spots". These areas were smoking/smoldering with no visible flames. This was potentially the result of spontaneous combustion, there were not any problems with mining machines, PPE, training or compliance issues.

The Wana Service Hoist primary counterweight became dislodged due to the guide rail becoming "sprung" and causing the rails to be out of gauge. There were no entrapments or injuries involved with the incident. It did not involve any compliance issues.

The Kuhntown elevator was transporting three employees to the surface when the safety switch on the surface door failed, causing the employees to become entrapped approximately 240' from the surface. Once the safety switch was replaced the employees exited the elevator at 1:34am. It did not involve any injuries, equipment or compliance issues.

Employee was operating a scoop loaded with a LW shield. EE lowered the bill but failed to release the winch. This caused the shield tip to raise. The shield tip caught the scoop canopy, caused it to raise and slide off the side of the scoop. The light caught the employee's hardhat forcing their head down and backwards. No compliance, PPE or protective device issues.

The hoist kicked the breaker 4' from the bottom as it was traveling down. While resetting power, employee attempted to jog the hoist before the system powered up. The PLC was not aware oil was still in the system, which prevented the brakes from properly setting and caused the hoist to drop to the bottom of the shaft. When the hoist dropped, three guide wheels were damaged.

Diagnosed as a abdominal strain (No hernia).
