Moving wooden pallet in shop caused lower back strain.
Exited the cab of a Hydraulic Shovel to use the restroom and the jacket of the employee snagged on the door and slammed on the employees left middle finger. Employee was seen at the ER and had X-Ray imaging done on the affected finger. X-Ray showed a hair line fracture on the affected finger. Employee received a return to work no restriction on next scheduled shift (11-25-2024)
Employee was unloading tools from tool box and putting them on a tool cart. Employee felt discomfort in the lower abdomen while pushing tool cart. Employee notified supervisor.
Employee was cleaning the welding area of the deployed geomembrane liner with their back to 2 other crew members who were rolling up a scrap piece of geomembrane material. The rolled up scrap geomembrane got away from the two crew members and contacted cleaning employee on back of ankle causing employee to fall to their knees.

Pinched finger when fuel hose slipped.
Employee was changing track pad and fractured/lacerated right thumb when pinched between 2 pads on dozer.
While moving drill sample bags the employee stumbled, twisting their back.
Installing pin on articulation lock. The pin slipped and pinched EE's right ring finger.

Warehouse tech receiving a shipment of Haul truck wheel chocks (roughly weighing 25 - 30 pounds) from a tote. Picking up last wheel chock felt, employee felt back tighten up, could not sit or stand without pain. Employee was treated at local ER, issued a full release back to work. Upon follow up appointment on June 18th, employee was unable to return to work.
Employee was helping change a bucket on a skid steer, overexerted and felt a pain in the left side of their groin. Employee taken to Urgent Care where it was determined a bilateral hernia. Employee released to comfort at normal duties. EE has not missed any work due to injury. Employee went to doctor on 3/21/2019 to schedule surgery.
Two employees were dismantling an elevated section of pipe from a catwalk. An employee was removing bolts from the pipe flange while the other held the other end up with one hand to alleviate pressure from the remaining bolts. When the second to last bolt was removed, the pipe swung down and pinched the employees left thumb between the flange and hand rail. 10 stitches.
Driller helper smashed and cut finger on the mud tank and hand rail.
Driller was changing a bit and when ee went to remove the bell housing off the bit. EE cut the top of right index finger on the bottom of the steel on a burr.
