While clearing brush, the employee got a wood shaving beneath their face shield and safety glasses, causing irritation to their left eye. Employee was evaluated by a physician and prescribed medicated eye drops. No lost workdays or restricted duty resulted.
The employee was manually operating a GOAB switch when an overhead porcelain insulator broke into multiple small pieces. One piece fell and knocked off the employee's hard hat.
