The employee was attempting to hook up a diesel fuel trailer to a diesel ride. While lining up the trailer hitch to the bumper of the ride the wooden prop that the trailer was resting on slipped, mashing EE's middle left finger between the hitch and the bumper causing a fracture and sutures

The employee was hooking up a cathead to a generator, employee was getting up off of the ground, their hand was on the edge of the hitch, their hand slipped on the side of the hitch it slid over a sharp burr causing a cut resulting in sutures

EE was picking up a 6"X20' section of waterline when EE felt a sharp pain in EE's lower back. EE was released to work on 03/12/2019, EE didn't begin losing time until 04/05/2018.

The contract employee had finished loading supplies on a man-trip when the employee bent down to lift lunch box. When employee lifted lunch box employee felt a pain in their back. The following were not a factor: rules, regulations, mining systems or equipment, training, protective clothing or devices. Attitude may have been a factor. Did not start to lose time until 3/2/2019.

The miner received 1st and 2nd degree burns when ee removed a battery lift jack after the bottom pin was cut out with a cutting torch. The miner placed the jack on the mine floor and the hot oil sprayed on the miners hand. The miner was wearing gloves at the time of the accident.
