The employee was attempting to hook up a diesel fuel trailer to a diesel ride. While lining up the trailer hitch to the bumper of the ride the wooden prop that the trailer was resting on slipped, mashing EE's middle left finger between the hitch and the bumper causing a fracture and sutures
The employee was changing bits on a continuous miner. The employee struck the tip of left little finger with a hammer causing a fracture to the tip end.
The employee attempted to step off of a spare belt tailpiece when EE put EE's foot on the ground, EE twisted EE's right knee. The incident occurred 10/14/2019 but did not start missing time until 02/03/2020

Operator reeled up bolter cable while tramming, EE was standing on the wrong side the cable when the cable was torn down from the roof striking EE in the jaw. EE was shoved against the rib causing EE to strike head causing a laceration to head resulting in 3 staples.
The employee was raising the drill pot to the roof EE was attempting to put the drill steel in the pot, the steel got caught on the chuck, when it went into the chuck it had too much up pressure the steel caught EE's glove and struck EE's hand. The employee did not start missing work until 8-3-2019.
The employee was cutting a metal band that was around a pad of concrete blocks, the band broke loose cutting the employee in their right arm, resulting in to receive sutures.

The contract employee had finished loading supplies on a man-trip when the employee bent down to lift lunch box. When employee lifted lunch box employee felt a pain in their back. The following were not a factor: rules, regulations, mining systems or equipment, training, protective clothing or devices. Attitude may have been a factor. Did not start to lose time until 3/2/2019.
EE was servicing man trips for the start of third shift, EE had just finished one and parked it in front of the bath house, when EE got out of the man trip and stepped on the gravel parking lot and twisted lower back. EE didn't start losing time until 03/18/2019.

The miner was riding on the 14 man Damacus man trip, the man trip ran over a skid located in the rides path. The ride came to an abrupt stop causing the miner to strike knee on the back of the seat in front of EE. The miner went to the ER and was released, the miner didn't start losing time until 01-04-2019.
EE was loading 5'resin in ee scoop bucket when ee felt a pain in left shoulder. EE had already loaded 20 bundles of glue before the incident.
The employee was bolting, had a bottom steel hung in the roof. The employee had 1' wrench in the drill pot and was raising it up to connect it with the steel, and caught left hand between the two, causing a laceration requiring sutures
The employee was operating the unit scoop. EE got out of the scoop for some unknown reason, the scoop started to roll. While trying to get back in the scoop, EE's right foot was caught between the coal rib and the scoop deck, resulting in a partial amputation of EE's right foot.
Our mantrip was coming off #5 road and was turning on to #4 road the mantrip did not make the turn, the operator slowed down to re adjust the turn, another mantrip came from behind and hit the back of our mantrip. The employee did not start missing work until 10/08/2018

Employee was between #2 & #3 entries, employee was attempting to break a hose loose and when it did break loose employee fell backwards tripping over the bolter tray landing on employee's elbow, jamming employee's left shoulder.
The Employee stated: Employee was using a utility knife to cut a piece of ventilation curtain and cut left hand
The Employee stated: that EE was bending a roof bolt on a flat surface, the roof bolt twisted around while EE was bending it and EE felt a strain on the right side of EE's lower back.
Employee stated; was unloading 20ft sections of 4 inch fresh water line on #2 unit. Employee was working alone when they stated that they felt pain in their lower back on the left side.
3 individuals were in the process of replacing the lid on the power center. The injured person was on one side of the lid and the other two were on the opposite side of the lid. As they were placing the lid in place two of them accidentally dropped their end. The injured employees hand was underneath allowing the lid to land on left index finger.
Employee Stated: EE was building a brattice. EE turned around in a twisting motion and bent over, picked up a block and raised up turned around and felt a pain in lower back on the right side.

Employee went to dentist and a crown on EE's tooth was broken. 4-15-2016 it was brought to my attention that a crown was broken and replaced on EE's teeth after seeing a dentist.
EE traveling out on damascus mac 10 mantrip employee failed to maintain EE's entire leg inside mantrip while traveling through airlock door and was struck by door causing seven stitches to left leg (shin).

Employee states while moving miner back out of cut dropped his head walking through a wing curtain and tripped over a pile of spilled coal or gob losing his balance, caught his self and didn't fall to mine floor, but twisted his back. Taken to emergency room. Employee also stated he has an ongoing back issue with disk deterioration.
Employee stated he was closing airlock door at the mouth of # 1 unit and timber worked loose and door fell striking him on his left hand on ring finger cutting at first joint of finger. Took to Ohio co. ER and received four stitches
Employee states on the 24th of October, dispatcher was getting out of her chair in the office and twisted her knee. Employee reported to safety department on the 29th of October and went to hospital from work and returned that date with no release until the 2nd of November. Employee states felt major pain and burning to knee.
Employee was on #3 unit entry #7 working on the pre-dump of double boom fletcher roof bolter. Employee told the operator to try the pr-dump. As employee was getting up off the mine floor he became off balance and grabbed the pr-dump with his left hand and the pre-dump closed on it resulting stitches to his middle finger and index finger.
Employee stated: bolting in #4 entry on #4 unit and had pinner wrench on bolt when boom was lowered, the wrench came out of the chuck striking his mouth resulting in a fracture to top left tooth.
Employee stated: was moving power on #2 unit and was in #6 entry moving power cables. There was mud and water in this entry and was dragging cables thru this area resulting in straining right side of lower back.
Employee stated: was on #1 unit entry #7 bolting the face. The steel was placed into the chuck when the glove was caught in between the base of the steel and the chuck. The rotation lever was pushed resulting fracture to the right wrist.
Employee stated: Installing top Roller belt framing on the #4 unit entry #4 when thumb on the left hand became caught between the framing and the rope resulting a fracture to the end of the left thumb.
Employee stated: was moving belt air lock on #3 unit entry #4 while bending over to pick up timber felt lower back knot up and could not straighten up.
Employee was cutting the plastic top off of a belt chain bucket with a belt knife. The knife slipped resulting in a cut on lower right leg. The cut did not require stitches, the cut became infected and went to the Dr. and received antibiotic to treat the infection.
Employee was in the shop on the surface bending a piece of metal in the bending press. While bending a piece of metal, the metal broke and struck right index finger resulting in a laceration in the bend of the finger. Employee received five stitches.
Employee was on #2 unit in the #3 entry working on a breaker panel lid. Employee was trying to remove pin from the breaker panel when a piece of a chisel struck and entered the top of his left hand.
Employee was on #4 unit entry #5 working on the Feeder. He put pin in the cat assembly and removed come along off the cat assembly. They lowered the cat assembly with the hydraulics of the feeder and as the assembly was coming down the cat caught the tip of the hook and slip off striking him in the mouth and busted one tooth.
Employee was on #4 unit in the #5 entry walking and stepped in a hole and felt right knee pop.
EE was bolting on the #1 unit entry #8 right. While putting steel in roof accidentally hit the rotation and steel stuck right hand resulting in fracture to the index finger.
EE was rock dusting on the #6 belt x cut #57. EE was getting out of the deck of the scoop. When all the body weight was placed on the left leg the left knee twisted.
EE was on #3 unit supply road loading old pallets onto a trailer. EE picked pallet up and twisted to throw pallet onto the trailer which resulted in a strain to the lower left side of his back.
#1 unit #7 entry in the last open cross cut. EE was loading glue on back of the roof bolter. While turning back around to put glue on the bolter, right side of head struck damaged pie pan on roof bolt. Piece of pie pan went under the side of hard hat causing laceration three inches above right ear resulting in two staples.
Was bolting on #3 unit entry #9 at the face. Made roof bolt up and walked to front of the bolter and turned to get bolt. When turning back around right shoulder struck pie pan resulting in laceration. Received 9 stitches.
A roof fall occurred on the #1 unit working section in the face of #5 entry measuring 5 feet high by 25 foot long by 16 foot wide. This area was bolted with 5 foot fully grouted roof bolts and 9 foot cable bolts.
EE was bolting on #3 unit entry # 0 at the face of entry. After making roof bolts up to install in the mine roof EE turned to walk to middle of bolter and twisted right knee.

Employee states while riding in on a mantrip to the unit they hit a bump jarring employees lower back
EE states: Joy miner seemed hung up, went to look operator side cat and miner caught traction on one side, end of tail hit the operator around shoulder area on right shoulder, left shoulder hit coal rib. Resulted in a fracture to the collar bone.
EE states: was throwing concrete block and strained left shoulder. This accident occurred 2/24/2014 as reported only. I was just notified yesterday that he had surgery thru Human Resources.

Employee came to me march 11, 2013 and reported a possible injury to his shoulder while building brattice. This was a report only, he missed no work his shoulder got worse and went to dr. and received surgery on sept 13, 2013. No days lost until day of surgery!
While flushing water line, the water hit him in left eye with mud and water.

EE STATED THAT HE WAS WORKING ON #2 UNIT BUILDING A BRATTICE. AFTER GETTING OUTSIDE AT THE END OF THE SHIFT NOTICED THAT HIS BACK WAS HURTING. NEXT DAY REPORTED TO ME STATING NEEDS TO GO TO DOCTOR.
WHILE WORKING ON #4 SECTION INSTALLING BELT, EE WAS THROWING 6X6 TIMBER UNDER THE FEEDER AND CAUGHT RIGHT THUMB BETWEEN FEEDER AND TIMBER. EE WAS TAKEN TO EMERGENCY ROOM AND RECEIVED SEVEN STITCHES.

EE WAS ROOF BOLTING ON # 3 SECTION AND ROCK FELL FROM OVER HEAD AND STRUCK ON BACK OF HEAD AND LEFT HAND. EE WAS TAKEN TO EMERGENCY ROOM IN DAVIS COUNTY AND RECEIVED FIVE STITCHES IN PALM OF HAND
Employee was working in battery charging station and cut his left ear on fire suppression system hanging from the Mine roof. Employee finished shift and went to local hospital after work and received five stitches inside ear.
while bolting on #2 section stood up from kneeling position and slipped on rock causing twisting of right knee.

UTILITY PERSON BUILDING BLOCK STOPPING ON RIGHT SIDE OF #2 UNIT, RAM CAR OPERATOR WAS NOT AWARE OF SITUATION AND SOUNDED BELL AND ATTEMPTED TO TRAVEL THRU CURTAIN STRIKING BLOCK WALL PUSHING IT OVER ON UTILITY PERSON ON OTHER SIDE. THE BLOCK FELL ON RIGHT LEG.
EE was loading eight inch water couplings on diesel ride and smashed his small finger on his right hand between couplings and steel post on ride. Transported to Ohio county received four stitches. After leaving hospital released to return to work same date.
Employee was pinning in #12 entry on #1 unit installing middle bolt (off side) in a six wide pin pattern and after setting ATRS to support the top he was standing and turned to his left and a large rock approx. 3'x 3'x 6"" broke and fell from the inby side of ATRS and fell down and rolled back on his right leg between knee and ankle.
ROCK FALL FOUND ON SLOPE AT KRONOS MINE AT SPAD # 1050+5 AND NOTIFIED MSHA BY PHONE JUST AFTER
EE WAS CARRYING METAL DECKING ON THE UPPER DECK OF SLOPE AREA AND DECKING SLID DOWN RIGHT SHOULDER CUTTING HIM ON RIGHT ARM BETWEEN ELBOW AND WRIST.
There was no witness to the accident, ee was struck in the face by the metal concrete pipe to pour concrete on the slope floor. Were cleaning out the excess concrete in the pipe at the time of the accident which includes inserting 2 foam balls into the top and blowing them down the pipe. When the pressure relapsed it appears to have kicked back.
A crane was overturned during a lift causing bruising to the left upper arm.
Employee was standing on crane when machine overturned resulting in contusions to multiple parts of the body.
