The employee was attempting to hook up a diesel fuel trailer to a diesel ride. While lining up the trailer hitch to the bumper of the ride the wooden prop that the trailer was resting on slipped, mashing EE's middle left finger between the hitch and the bumper causing a fracture and sutures

Operator reeled up bolter cable while tramming, EE was standing on the wrong side the cable when the cable was torn down from the roof striking EE in the jaw. EE was shoved against the rib causing EE to strike head causing a laceration to head resulting in 3 staples.

The following were not a factor: rules, regulations, mining systems or equipment, training, protective clothing or devices. Attitude may have been a factor. The miner was riding on the 14 man Damacus man trip, the man trip ran over a skid located in the rides path. The ride came to an abrupt stop causing the miner to strike knee on the back of the seat in front of EE. The miner went to the ER and was released, the miner didn't start losing time until 01-04-2019.

EE was loading 5'resin in ee scoop bucket when ee felt a pain in left shoulder. EE had already loaded 20 bundles of glue before the incident.

The employee was bolting, had a bottom steel hung in the roof. The employee had 1' wrench in the drill pot and was raising it up to connect it with the steel, and caught left hand between the two, causing a laceration requiring sutures

The employee was operating the unit scoop. EE got out of the scoop for some unknown reason, the scoop started to roll. While trying to get back in the scoop, EE's right foot was caught between the coal rib and the scoop deck, resulting in a partial amputation of EE's right foot.

Our mantrip was coming off #5 road and was turning on to #4 road the mantrip did not make the turn, the operator slowed down to re adjust the turn, another mantrip came from behind and hit the back of our mantrip. The employee did not start missing work until 10/08/2018

Employee put too much pressure on the pinner steel causing it to bend, when the steel was rotated the steel hit the operator in the left eye. The employee was fully released to duty, and didn't start losing time until 9-24-2018. The employee suffered a hand injury at home after the eye injury and was off from 09-17-2018 until 9-21-2018.

3 individuals were in the process of replacing the lid on the power center. The injured person was on one side of the lid and the other two were on the opposite side of the lid. As they were placing the lid in place two of them accidentally dropped their end. The injured employees hand was underneath allowing the lid to land on left index finger.

EE was loading eight inch water couplings on diesel ride and smashed his small finger on his right hand between couplings and steel post on ride. Transported to Ohio county received four stitches. After leaving hospital released to return to work same date.

There was no witness to the accident, ee was struck in the face by the metal concrete pipe to pour concrete on the slope floor. Were cleaning out the excess concrete in the pipe at the time of the accident which includes inserting 2 foam balls into the top and blowing them down the pipe. When the pressure relapsed it appears to have kicked back.
