Showing all 35Employee was moving rock on the back side of the rock box of the jaw crusher. EE's left pinky finger was caught between rock and the rock box. Laceration on outside of left pinky.
Employee was removing flip gates from bypass chute. Right thumb was pinched between the side liner and flip gate.
Employee was changing the bucket on the skid steer. EE was using foot as a brace while pulling on the bucket to latch it into place. The bucket slipped before it was latched and smashed top of left foot and big toe area.
Taking a block of wood out from under some angle iron on a trailer. Finger got smashed between a piece of angle iron and the trailer. Tip of middle finger on left hand was torn off to the bone(nail was torn off), tip of finger was broken.

Employee was cleaning up debris (dirt, dust) around the jaw crusher with a flat shovel for about 20 minutes. The next day he started complaining about back pain. The following day (March 23/2011)he came to work with the same problem and was sent to see a doctor.
Employee was getting ready to pick up 200 lb shaft with crane when another contractor pulled on sling to tighten it. While employee was looking up at the hook being lowered, shaft rolled off stand and landed on right foot behind steel toe. Other contractor didn't speak up to alert employee.
IE was crawling under gyratory crusher twisting and bending to perform maintenance. Later that night IE noticed knee was swollen. He wrapped in ice. On 4/20/10, IE was working on a jaw crusher and his knee began to hurt again. He noticed fluid build up under the knee cap. On 4/29/10, he went to doctor who drained the fluid and cleaned out area around joint.
Plant personnel were loading jaw dies onto pickup truck, for return to Warfordsburg, using a wheel loader with a chain. Employee tried to steady swinging jaw die, when it pushed against straightened left arm and caused cracking sound in left shoulder. A small amount of pain immediately appeared in his left shoulder.
Employee was bent over parts washer rack and was getting ready to lift starter from rack. As he picked up on the starter, he sneezed and felt a pop in his belly area. He has been diagnosed with an umbilical hernia. A 2-ton job hoist is available to be used to lift items out of the parts washer and employee has been instructed to use it more frequently during all lifting.
Employe was holding a piece of sheet metal while working on a piece of equipment. He lost his grip and the metal cut his left pinkie finger.
Employee was collecting a test sample of stone and lost control of the sample tray she was holding. The tray came down on her wrist, pinching it against a metal railing. A permanent access platform has been installed for easier access and will be modified to reduce the risk of injury.
Employee was performing preventive maintenance on a haul truck. She then experienced severe pain in the lower left side of her back.
EMPLOYEE OPENED AN INSPECTION DOOR APPROX 24" X 24" IN SIZE, IN ORDER TO SEE WHY THE MACHINE WASN'T OPERATING PEOPERLY. WHILE WORKING ON THE MACHINE, THE UNSECURED DOOR FELL ON HIS RIGHT HAND AND WRIST.
EE WAS BENDING OVER TO TURN ON WATER VALVES & TWISTED HIS BACK.

EE WAS TIGHTENING A CHAIN BINDER TO SECURE A LOAD FOR TRANSIT. THE CHAIN BINDER SLIPPED, HIT THE EE ABOVE HIS RIGHT EYE, CAUSING A LACERATION.
WHILE REMOVING A TOP DECK SCREEN FROM A 6 X 20 SHAKER, THE SCREEN CLOTH DROPPED, PINCHING EE'S FINGER ON THE SECOND DECK DISCHARGE LIP.
EE WAS STRUCK BY A FALLING OBJECT THAT FELL APPROX. 2 FT AND STRUCK HIS R LEG INT HE THIGH AREA. HE WAS HANGING A 9 1/2"X 92 1/2". PIECE OF AR THAT WAS FASTENED WITH 2 BOLTS ON EACH END. WHEN HE ATTEMPTED TO BEND OVER TO PICK UP A NUT, HE LOST GRIP ONT HE LINER AND IT FELL STRIKING HIS LEG.
EE WAS REPAIRING A PIECE OF EQUIPMENT USING GUN CUTTER. A WHILE LATER HE FELT TINGLING IN HIS RIGHT LITTLE FINGER, NOTICING SOME SWELLING & REDNESS. WASHED HIS HANDS & LATER HAD A RED STREAK G OING UP WRIST & TOP OF HAND. HE APPARENTLY HAD A SMALL SCRATCH THAT THE CHEMICAL SEEPED INTO.
WHILE ATTEMPTING TO INSTALL COUNTERWEIGHTS ON SCREEN GEAR DISC, THEY SLIPPED AND LANDED ON LEFT PINKY FINGER. SMASHING LEFT PINKY FINGER AND LOSS OF FINGERNAIL.
WHILE ASSISTING ANOTHER EE TO MOVE A SCREEN, 100-150 LBS HE FELT PAIN ON HIS LEFT SIDE NEAR HIS BELT AREA THAT MOVED DOWN INTO THE GROIN AREA.
LIFTING PUNCH PLATE ONTO THE BACK OF THE CREW CAB TRUCK. SOMETHING CAUSED PAIN/NUMBNESS TO OCCUR IN HIS LEFT ARM/DOWN HAND.
EMPLOYEE WAS BEING ASSISTED BY ANOTHER EE WHILE LOADING 9' LONG CROSSTIES INTO 988F LOADER BUCKET. ONE EE HELPING UNEXPECTEDLY DROPPED HIS END OF CROSSTIE CAUSING EE TO LOOSE HIS GRASP ON HIS END AND THE CROSSTIE DROPPED LANDING ON HIS RIGHT FOOT.

REBUILDING SCREEN; BEFORE CLOSING MACHINE HE MANUALLY WAS SPINNING SHAFTS WHEN HIS GLOVE BECAME CAUGHT BETWEEN 2 COUNTER WEIGHT, PULLING HIS RIGHT HAND IN. HE HAS A BLOOD BLISTER AND SWELLING ONHIS RIGHT RING FINGER.
WHILE STANDNG ON A 8'LADDER INSTALLING CURVED SHEET METAL AND PANEL. THE PANEL WAS BEING LOWEREDINTO PLACE, LINE UP BARS WERE INSTALLED AND PANEL COULD NOT BE HANDLED BY HIS END, IT SLIPPED CO MING DOWN AND STRUCK HIM ON RIGHT ARM.
